Job summary

Allscreens Nationwide Ltd is seeking an experienced Medicines Management professional to review hospital initiated medication changes in line with Practice Protocols and update patient records accordingly.

You will support and coach administrative staff, contribute to Medicines Management Policy development, review Repeat Prescribing Protocols to improve efficiency, advise GPs and staff on prescribing changes, and help ensure safe, cost-effective prescribing.

Qualifications

• Experience in medicines management within primary care or hospital settings.
• Knowledge of prescribing policies and safety practices.
• Strong communication with GPs, staff and patients.

Responsibilities

• Review hospital initiated medication changes per Practice Protocols and update patient records.
• Support and coach administrative staff preparing repeat prescriptions to follow Policies.
• Contribute to Medicines Management Policy development to meet clinical targets.
• Review Repeat Prescribing Protocols to improve efficiency and safety.
• Advise GPs and staff on prescribing changes to improve safety, quality and cost.
• Modify patient prescribing records to support safe, cost-effective prescribing.
• Assess risk and recommend improvements for safe handling of prescriptions.
• Contribute to SEAs where medicines management is central.
• Respond to medication queries from patients and staff with appropriate GP escalation.
• Advise on sourcing and safe management of medicines as appropriate.
• Assist in controlling medicine stocks within the Practice.
• Liaise with agencies/colleagues/patients regarding current advice as applicable.

Job responsibilities

• To review hospital initiated medication changes in accordance with Practice Protocols and to update Patients medical records / prescribing accordingly.
• To support and coach administrative staff involved in the preparation of repeat prescriptions in order to ensure that Practice Polices and Protocols are adhered to.
• To contribute to the development and implementation of a Medicines Management Policy in order to support the achievement of the Practices prescribing Quality and Financial Targets.
• To contribute to the review and development of Repeat Prescribing Protocols / Standard Operating Procedures to improve the efficiency and effectiveness of Repeat Prescribing systems.
• To provide advice to GPs, staff and patients in changes to prescribing in order to support the improvement of prescribing safety, quality and cost effectiveness.
• To make changes to Patients prescribing records in accordance with Practice Protocols in order to support safe and cost effective prescribing.
• To take a proactive approach to the safe handling of prescriptions, including assessment of risk and making recommendations for improvement.
• To contribute to SEAs where medicines management is a central factor
• To respond to medication queries from Patients and Staff in a professional manner, referring to the appropriate GP in accordance with Practice Protocols.
• To advise on the sourcing and safe management of medicines as appropriate.
• To assist in the control of medicine stocks within the Practice.
