Job Description: To work under the supervision of the Pharmacist to ensure the safe, accurate and timely supply of prescriptions to patients.

- Complete audits around patient safety and cost effective prescribing and present the information for sharing with the clinical team.
MAIN DUTIES AND RESPONSIBILITIES
- Work with the Pharmacist and Clinicians in accordance with agreed practice prescribing standard operating procedures.
- Process repeat medication requests, including the conversion of acute requests to repeat medicines, changing quantities of medicines.
- Issue prescriptions in line with agreed clinical protocols and within your competency
- Liaise with secondary and primary care colleagues to ensure correct medicines are continued following the transfer of care.
- Monitor practice adherence to the repeat prescribing policy
- Assist the Pharmacist with data analysis of prescribing behavior in line with NICE guidance and local clinical commissioning directives.
- Assist with the development and review of medicine audits
- Maintain and develop medicines communication to staff
- Provide a contact for patients and colleagues with medication queries.
- Use practice and CCG templates to record near patients monitoring for high risk drugs.
- Identify blood tests, recalls and reviews using established protocols and forward these to the appropriate clinician.
- Liaise with Clinicians and staff to resolve prescribing queries.
- Review hospital discharge notifications and outpatient hospital letters and reconcile medication against current repeats, flagging up and correcting discrepancies.
- Promote safe use of medication, reporting of medication related incidents and proactively preventing safeguarding incidents.
- Observe the principles of clinical governance when developing policies, protocols, guidelines, SOPs and procedures.
- Support practice prescribing budgets by on-going monitoring, audit and analysis of drug use and support practices in reviewing, developing and enhancing cost-effective prescribing.
- Help review medication with the Pharmacists and make recommendation for prescribing changes to improve outcome and ensure prescribing of the most effective product.
- Project work/prescribing initiatives under the support of the Pharmacist
- Adhere to strict confidentiality policies at all times.
- Attend local meetings seeking to improve access to medicines or repeat prescribing processes.
- Undertake audits of prescribing as agreed by pharmacist
