Job description

Arbennek PCN is looking for an enthusiastic and highly motivated person to join its team as a Pharmacy Technician.

The successful candidate will work with Clinical Pharmacists along with the practice and PCN's wider multi-disciplinary team, under supervision, to ensure the safe, accurate and timely supply of prescribed medication to patients.

Main duties of the job
  • Support Clinical Pharmacists at practice and PCN level with medicines optimisation and patient care initiatives.
  • Perform medication switches and updates in GP practices in line with agreed protocols.
  • Report on key performance indicators for CPGP to demonstrate added value for patients and general practice in accordance with NICE guidance.
  • Conduct and share outcomes from clinical audits using SystmOne.
  • Monitor MHRA drug alerts and help implement appropriate medication changes.
  • Use and maintain information systems and databases relevant to the position including incident reporting, medicines optimisation, prescribing data, cost savings and patient outcomes.
  • Collaborate with GP practices, ICB teams, community and hospital pharmacies.
  • Participate in meetings, including PCN Multi Disciplinary Team meetings.
  • Help deliver the wider Medicines Optimisation Strategy.
  • Handle sensitive information professionally, in line with the Data Protection Act and Caldicott Guidelines.
  • To be responsible for the organisation, planning and of own workload to meet set deadlines.

Arbennek PCN is located in the central ICA within the Cornwall and Isles of Scilly Integrated Care System and has approximately 32,453 people registered from 4 GP Practices Brannel Surgery, Clays Surgery, Probus Surgery and Roseland Surgeries.

Job responsibilities

Undertakepatient facing and patient supporting roles to ensure effective medicines use,through shared decision-making conversations with patients.

Carry outmedicines optimisation tasks including effective medicine administration (e.g.checking inhaler technique), supporting medication reviews and medicinesreconciliation. Where required, utilise consultation skills to work inpartnership with patients to ensure they use their medicines effectively.

Asdetermined by the PCN, support medication reviews and medicines reconciliationfor new care home patients and synchronising medicines for patient transfersbetween care settings, linking with local community pharmacies

Supportthe Clinical Pharmacist in Structured Medication Reviews (SMR) i.e. organise necessary monitoring tests prior to SMR

Provideexpertise to address both the public health and social care needs of patients, including lifestyle advice,service information, and help in tacklinglocal health inequalities.

Manageshared care protocols and liaise with Clinical Pharmacists for more complex patients.

Supportinitiatives for antimicrobial stewardship to reduce inappropriate antibioticprescribing locally.

Supportthe PCN multi-disciplinary team to ensure efficient medicines optimisationprocesses are being followed.

Implementefficient ordering and return processes and reducing medication wastage.

Providetraining and support on the legal, safe and secure handling of medicines,including the implementation of the Electronic Prescription Service (EPS).

Promotion of Electronic Repeat Dispensing(eRD) and online ordering

Develop relationshipswith other pharmacy professionals and members of the multi-disciplinary team tosupport integration across health and social care including primary care,community pharmacy, secondary care and mentalhealth.

Supportpractice reception teams in streaming general prescription requests, so as to allow GPs andclinical pharmacists to review the more clinically complex requests.

Supportthe implementation of national prescribing policies and guidance within GPpractices, care homes and other primary caresettings.

Supportthe PCN to deliver on QIPP agenda, QOF and locally commissioned enhanced services

Supportingquality improvement measures and contributing to the Quality and Outcomes Framework and enhanced services;

Supportthe PCN in reviewing and developing practice policies for CQC requirements

Experience
  • -Evidence of continued professional development (CPD)
  • -Experience of working as a qualified, registered pharmacy technician in primary care, community or hospital pharmacy
  • -Demonstrate ability to influence and persuade partners and stakeholders of the respective merits of different options, innovations, new opportunities and challenges
Qualifications
  • -Professional registration with GPhC
  • -Meets the specific qualification and training requirements as specified by the GPhC criteria to register as a Pharmacy Technician
