Job summary

Manchester University NHS Foundation Trust (MFT) is seeking a dedicated pharmacy technician to support community urgent care services, including domiciliary medicines optimisation and MDT collaboration. You will work within the CMOS framework to ensure safe and effective patient care across Manchester.

You must hold GPhC registration and a Pharmacy Technician qualification; prior hospital or community pharmacy experience is essential.

Job Summary

Manchester Community Urgent Care services

This is a fantastic opportunity for an enthusiastic and motivated pharmacy technician to join the well supported and award-winning Manchester and Trafford Local Care Organisation, community medicines optimisation service (CMOS). The service provides patient facing clinical pharmacy services, working closely with other pharmacy teams and services across Manchester Foundation Trust.

The LCO Urgent Care Crisis Response and Discharge to Assess (D2A) services play an important role in supporting the NHS 10-year plan to deliver more care in patients' homes and community settings. The D2A service supports individuals recently discharged from hospital to remain safely at home. The Crisis Response service provides timely, responsive care to patients at home, helping them avoid unnecessary attendance at hospital emergency departments and other healthcare services. This model of care is delivered by an experienced multidisciplinary team (MDT), including advanced care practitioners (ACPs), nurses and pharmacists.

To apply for this post, you must be a General Pharmaceutical Council (GPhC) registered pharmacy technician with substantial experience of working within primary or secondary care. You should be confident to work independently and have excellent organisational and interpersonal skills. You must have a full UK driving licence and access to a car for work, as domiciliary visits are an important part of this role.

Main duties of the job

The pharmacy technician is responsible for providing input as part of the medicines support to community urgent care service. Domiciliary visits to patients for medicines optimisation interventions is a key feature of this role as well as multidisciplinary (MDT) working.

• Supporting the delivery of medicines optimisation to Discharge to Assess and Crisis response.
• Supporting pharmacists to establish safe working arrangements for medicines in the Community Medicines Optimisation Service.
• Liaising with pharmacists, technicians and other healthcare professionals in secondary and primary care to ensure continuity of care on patient transfer.
• Carrying out medication and compliance reviews with patients in area of clinical competence
• Identifying and act on medication errors/queries within area of competence, reporting complex issues to a medicines optimisation pharmacist as appropriate.
• Providing support to other CMOS workstreams as necessary.

The Costs for a DBS

At Manchester University NHS Foundation Trust (MFT), roles that require a DBS check are subject to a cost recovery policy. This means that the cost of submitting and processing a DBS application if you are successful and join us, will be deducted from your salary within the first three months of employment. If, however, you withdraw from a conditional offer after the DBS process has begun, you will still be liable for the cost incurred.
