Job description

We are looking for anenthusiastic Pharmacy Technician to join our expanding medicines managementteam at Wide Way Medical Centre. We currently employ 1 Pharmacy Technician and1 Clinical Pharmacist and are recruiting a second Clinical Pharmacist to theteam, but recognise we need to expand to meet the needs of our 16,000population.

We serve a diverse population ina relatively deprived area of Merton with a high burden of cardiovasculardisease and related chronic disease in particular.

Main duties of the job

The Pharmacy Technicians willassist the Clinical Pharmacists with a range of medicines management activities i.e. medicine related policies, protocols and procedures, their communication and implementation under the guidance of the partnership. This includes:

- Implementing policies,protocols and procedures pertaining to the prescription of medication by WideWay Medical Centre staff

- Implementing repeat medication procedures,liaising closely with non-clinical staff

- Reconciling medicines forpatients who are started on new medication and/or whose medication is changedby other healthcare providers

- Conducting annual reviews ofmedication for specified cohorts of patients (SMR) within their scope ofpractice

- Ensuring medication monitoring iscompleted, carrying out medication safety audits and responding to safetyalerts

- Ensuring procedures for theissuing, delivery and claiming for Personally Administered Items (PAIs) arefollowed, liaising with clinical staff and the nursing team who administer them

- Liaising with local communitypharmacists to build strong collaborative relationships with regular meetingsto ensure efficient collaborative working

- Liaising with ICS Place and PCNpharmacists to deliver on national, regional and local priority initiatives

- Helping to answer internal andexternal medication-related queries within their scope of practice

- Delivering onmedication-related Quality Improvement activities

Wide Way Medical Centre is based in South West London.

We take care of almost 16000 patients in a modern, purpose built surgery with ample car parking.

Our team consists of 4 GP Partners plus a Managing Partner, 5 Salaried GPs, 2 Advanced Care Practitioners, Practice Nurses, a Paramedic, Social Prescriber, Health and Wellbeing Coach, Care Coordinators and a fantastic administrative support team.

We are a training practice with 4 GP trainers and we host GP registrars, medical students and nursing students.

the successful candidate will take part in the weekly cardiovascular optimisation meetings, planned care meetings and medicines management meetings to support the medicines management team

Job responsibilities

• Support the repeat prescribing process, ensuring prescriptions are issued accurately and in line with clinical guidance.
• Conduct medication reconciliation after hospital discharge or clinic appointments, liaising with clinical staff as needed.
• Assist in the implementation and monitoring of medicines safety protocols (e.g. high-risk drugs, controlled drugs).
• Manage and maintain accurate patient medication records.
• Process prescription queries from patients, community pharmacies, and other stakeholders.
• Monitor and update drug monitoring alerts, working closely with clinicians and clinical pharmacist to ensure patient safety.
• Assist in identifying and managing patients suitable for repeat dispensing.
• Support the implementation of prescribing audits and quality improvement initiatives.
• Work with the practice team to reduce medication waste and promote cost-effective prescribing.
• Help coordinate and follow up on medication reviews, including sending invitations and updating records.
• Communicate relevant medicines-related information and changes to practice staff and patients.
• Liaise with community pharmacies, PCN and ICS pharmacy teams to ensure continuity of care.
• Participate in clinical and operational meetings, contributing to medicines optimisation planning.
• Support the delivery of national and local prescribing targets, such as QOF and IIF.
• Maintain up-to-date knowledge through continued professional development in line with GPhC requirements.

Person Specification

Experience

• Proven post-qualification experience in a community, hospital, or primary care setting
• Experience with medicines reconciliation and repeat prescribing processes
• Familiarity with electronic prescribing systems (e.g. EMIS)
• Experience working in a GP practice or PCN
• Knowledge of QOF, IIF, or other NHS performance frameworks
• Experience of working with multi-disciplinary teams (MDTs)

Knowledge

• Sound understanding of medicines management, medication safety, and prescribing protocols
• Strong IT skills, including clinical systems and Microsoft Office
• Knowledge of drug monitoring requirements (e.g. blood tests for DMARDs, lithium)
• Ability to follow and implement clinical and administrative protocols
• Understanding of local/national medicines optimisation strategies
• Awareness of NHS structures and initiatives in primary care

Qualifications

• Registered Pharmacy Technician with the General Pharmaceutical Council (GPhC)
• Evidence of continuous professional development (CPD
• Completed or working towards accuracy checking technician (ACT) qualification
• NHS/Primary Care specific training (e.g. CPPE Primary Care Education Pathway)
