Qualifications

  • Registered as a Pharmacy Technician with the General Pharmaceutical Council.
  • BTEC or NVQ level 3 in Pharmacy Services or equivalent is necessary.
  • Previous experience as a Primary Care Pharmacy Technician preferred.

Responsibilities

  • Liaise with patients and healthcare professionals regarding medication reviews.
  • Conduct medicines reconciliation and identify patients for medication changes.
  • Participate in high-risk drug monitoring and support local clinical governance.

Job description

We have an exciting opportunity fora Band 5 Pharmacy technician to join our team of 10 Pharmacists and PharmacyTechnicians in Central Liverpool Primary Care Network.

We provide medicines managementsupport to 6 practices with central Liverpool and our PCN looks after apopulation of 45,000 patients.

We work in practices and in withinthe PCN as part of multidisciplinary teams to provide the bast care to ourpopulation.

Main duties of the job

The main aspects of the role include medicines reconciliation, high risk drug monitoring, medication audits, quality improvement projects and medicines education.

Job responsibilities

To liaise with patients, practice staff, GPs, care home staff andcommunity pharmacists in all aspects of medication reviews and prescribingqueries to improve the pharmaceutical care of patients.

To support medication review and conduct medicines reconciliation.

To identify patients suitable for medication changes in line withthe Medicines Management work plan to support QOF, prescribing targets andICB projects and subsequently make those changes with the agreement of theGP or pharmacist, e.g: dose optimisation, formulary switching.

To assess prescribing data using practice clinical computer systemsand patient records, highlight any recommendations and implement changesif required, to ensure patients medication records are up to date

Under the direction of the PCN Pharmacist/GPs, assist withmedication changes from hospital letters and medicines reconciliation fornew patients. To highlight any medication errors/queries and report anyconcerns to the PCN Pharmacist/GPs to support local clinical governancestructures

To implement initiatives to reducewaste, improve medicines compliance and ensure best practice is adopted inaccordance with local guidelines, legislation, safety updates e.g NICE, MHRA

To assist the PCN Pharmacist with any searches, data collection andletter production required for prescribing changes or drug alerts.

To facilitate High Risk Drug Monitoring

To provide information and answer technical medicines managementqueries from healthcare professionals and members of the public.

To help develop and implement medicines management initiativesthrough the work of the practice e.g. repeat prescribing, pharmacy MURs.

In conjunction with the PCN pharmacist deliver training andeducation on prescribing and therapeutics in general practice

To provide pharmacy systems training to new members of the team andprovide leadership to new staff.

To take an active part in our Covid Vaccination programme whenneeded, reconstituting vaccine, managing the cold chain and stock control

To be able to work as part of a team

To be able to work independently and have good time managementskills

To support and/or manage PCN wide projects depending on experience
