A healthcare provider in Hartlepool is seeking a Primary Care Network Pharmacy Technician to join their multidisciplinary team. This role includes supporting patients in medication management, conducting structured medication reviews, and collaborating with various healthcare professionals to improve patient care. The successful candidate will hold a relevant qualification and be registered with GPhC, demonstrating excellent communication and teamwork skills.

Pharmacy technicians play an important role, complementing clinical pharmacists, community pharmacists and other members of the PCN multi-disciplinary team. Pharmacy technicians work under supervision to ensure effective and efficient use of medicines and the core responsibilities will cover clinical, technical and administrative categories.
The purpose of the role is to lead improvements to maximise safe, cost effective best practice in prescribing and toimprove the quality of patient care.
The post holder will help patients to get the best from their medicines by switching medications to agreed and approved protocols, improving repeat prescribing processes in General Practice, including promotion of repeat dispensing and online ordering, minimising clinical risk and aiming to reduce wasted medicine.
The post holder will undertake a patient facing supporting role to ensure effectively medicine use, carry out medicine optimisation tasks.
They will support the clinical pharmacist in Structured Medication Reviews and provide expertise to address both the public health and social care needs to patients, including lifestyle advice and help in tackling local health inequalities.
Hartlepool Network are a team of 5 practices consisting of West Quay Practice, West View Millennium Surgery, Seaton Surgery, Gladstone House Practice and Hart Medical. Together we have approximately 35000 patients spread across our locality of Hartlepool.
Our Clinical Director and wider teams of Nurse Practitioners, Practice Nurses and administrators, along with our Additional Reimbursable Role (ARRS) staff of Social Prescribers, Care Co-ordinators, First Contact Physio, Mental Health team and our Pharmacist and Pharmacy technicians team work in harmony to deliver outstanding medical services to our patient population.
Clinical
Undertake patient facing andpatient supporting roles to ensure effective medicines use, through shareddecision-making conversations with patients.
Carry out medicines optimisationtasks including effective medicine administration (e.g. checking inhalertechnique), supporting medication reviews and medicines reconciliation. Whererequired, utilise consultation skills to work in partnership with patients toensure they use their medicines effectively.
As determined by the PCN, supportmedication reviews and medicines reconciliation for new care home patients andsynchronising medicines for patient transfers between care settings, linkingwith local community pharmacies
Support the Clinical Pharmacist in Structured Medication Reviews (SMR)
Provide expertise to address boththe public health and social care needs ofpatients, including lifestyle advice, service information, and help in tackling local health inequalities.
Manage shared care protocols andliaise with Clinical Pharmacists for more complex patients.
Support initiatives for antimicrobialstewardship to reduce inappropriate antibiotic prescribing locally.
Technical and Administrative
Support the PCN multi-disciplinaryteam to ensure efficient medicines optimisation processes are being followed.
Implement efficient ordering andreturn processes and reducing medication wastage.
Provide training and support onthe legal, safe and secure handling of medicines, including the implementationof the Electronic Prescription Service(EPS).
Promotion of Electronic Repeat Dispensing (eRD) and online ordering
Develop relationships with otherpharmacy professionals and members of the multi-disciplinary team to supportintegration across health and social care including primary care, communitypharmacy, secondary care and mental health.
Support practice reception teamsin streaming general prescription requests,so as to allow GPs and clinical pharmacists to review the more clinicallycomplex requests.
Support the implementation ofnational prescribing policies and guidance within GP practices, care homes andother primary care settings.
Support the PCN to deliver on QIPPagenda, QOF and locally commissioned enhancedservices
Supporting quality improvementmeasures and contributing to the Qualityand Outcomes Framework and enhanced services;
Support the PCN in reviewing anddeveloping practice policies for CQCrequirements.
Undertake monitoring searchesincluding CQC performance and safety searches in line with the needs of thepractices or PCN
Undertake duties as per agreed jobplan.
