A healthcare organization in the UK is seeking a registered Pharmacy Technician to join its team. An exciting opportunity for a registered Pharmacy Technician to join the Pharmacist Team at One Thornton Heath PCN to develop and support medicines optimisation across ourwell-established primary care network of six GP practices in North Croydon.
The role requires participation in all aspects of patient care through knowledge gained from training, working for One Thornton Heath PCN across three GP practices as part of their multidisciplinary healthcare professional teams and will be supported and supervised by a GP Supervisor. This is a key position within our developing PCN and involvement in routine care including care homes ensures the post holder has the necessary breadth of training and ongoing clinical challenge.

One Thornton Heath Primary Care Network is an establishednetwork of 6 GP practices with many years' experience of collaborative working.
Allthe practices are committed to providing high quality care for the 54,000registered patients within the PCN. This new post will join the team of 1THPCN staff currently working successfully across the three Thornton Heath practices.
Thepost holder will be employed by a lead training practice on behalf of the PCNand the lead practice will provide regular support and clinical supervision,reporting to the PCN Clinical Directors and Business Lead

Job Summary:
Therole requires participation in all aspects of patient care through knowledgegained from training, working for One Thornton Heath PCN across six GPpractices as part of their multidisciplinary healthcare professional teams andwill be supported and supervised by a GP Supervisor. This is a key position within our developingPCN and involvement in routine careensures the post holder has the necessary breadth of training and ongoingclinical challenge.
Job Responsibilities:
·Undertake patient facing and patient supporting roles to ensureeffective medicines use, through shared-decision making conversations withpatients.
· Carry out medicinesoptimisation tasks including effective medicine administration (e.g. checkinginhaler technique), supporting medication reviews, and medicinesreconciliation. Where required, utilise consultation skills to work inpartnership with patients to ensure they use their medicines effectively.
· Support, as determined bythe PCN, medication reviews and medicines reconciliation for new care homepatients and synchronising medicines for patient transfers between caresettings and linking with local community pharmacists.
· Provide specialistexpertise, where competent, to address both the public health and social careneeds of patients, including lifestyle advice, service information, and help intackling local health inequalities.
· Take a central role in theclinical aspects of shared care protocols and liaising with specialistpharmacists for more complex patients.
· Support initiatives forantimicrobial stewardship to reduce inappropriate antibiotic prescribing.
· Assist in the delivery ofmedicines optimisation and management incentive schemes and patient safetyaudits.
· Support the implementationof national prescribing policies and guidance within GP practices, care homesand other primary care settings. This will be achieved through undertakingclinical audits, supporting quality improvement measures and contributing tothe Quality and Outcomes Framework and enhanced services.
