Responsibilities

• Undertake medicines reconciliation following discharge or correspondence from secondary care, community services, and clinics.
• Maintain and optimise repeat medication lists, including synchronisation, review date management, electronic repeat dispensing, and appropriate amendments under agreed protocols.
• Support care home and nursing home patients through medicines optimisation, prescription changes, and liaison with clinicians and community pharmacies.
• Deliver patient-facing clinics where appropriate, including medicines optimisation, secondary prevention, cardiovascular disease (CVD) support, and frailty reviews.
• Undertake clinical support activities including observations (e.g. blood pressure), urine dip testing and, if required, phlebotomy (training provided).
• Support antimicrobial stewardship, opioid de‑prescribing, safety alerts, and the implementation of national and local prescribing guidance.
• Contribute to prescribing safety through audits, quality improvement activity, monitoring of recalls, blood tests, and medication reviews.
• Support effective stock management, prescribing data analysis, cost‑effective prescribing, and waste reduction initiatives.
• Promote safe use of medicines, report and prevent medicines‑related incidents, and elevate safeguarding concerns appropriately.

Job description

This is an exciting opportunity for the right candidate to join our existing team of Pharmacists and Pharmacy Technicians, working within the GP Surgeries that form Wakefield North Primary Care Network (PCN). The current team, led by our Lead Pharmacist and Lead Pharmacy Technician, have built strong and valued working relationships amongst themselves, other roles within the PCN and our GP practices as well as colleagues from a neighbouring PCN., Manage repeat prescription processes, including resolving queries, maintaining accurate medication records, and working from prescribing ledgers in a timely manner.

Wakefield North Primary Care Network (PCN) is seeking to recruit an additional Pharmacy Technician to join its expanding and well‑established pharmacy team. The current team have built excellent working relationships between themselves, with colleagues across the PCN practices and neighbouring PCN, fostering strong professional relationships and a supportive working environment. We provide clear pharmacy leadership, regular training, and actively support ongoing professional development by responding to the needs of the team.

Our PCN consists of six GP practices across the eastern area of Wakefield District (Henry Moore Clinic, Riverside Medical Centre, Tieve Tara Medical Centre, Station Lane Medical Centre, St Thomas Road Surgery, and Newland Surgery). where we all believe our patients should have the same opportunities of a happy and healthy life.
