Pharmacy Care Coordinator Mid Chiltern Primary Care Network

Primary Duties and Areas of Responsibility:

• Supporting clinical pharmacists and pharmacy technicians by booking patients into various pharmacy clinics, such as blood pressure, asthma, diabetes, medication monitoring, and medication reviews.
• Handling tasks that come into the shared mailbox.
• Booking patients for COVID and flu vaccinations.
• Managing clinics throughout the year for the team.
• Supporting patients with learning disabilities by maintaining contact and inviting them for their annual reviews.
• Managing cancer review recalls and dealing with invites for 3-month and 12-month reviews.
• Handling bowel screening and breast screening DNA (Did Not Attend) cases.
• Managing new diagnosed patients.
