Job Summary:

An exciting opportunity awaits for an enthusiastic pharmacist to join our friendly and progressive medicines management service, serving the community in Ealing. The role involves leading the medicines management service for the Rapid Response Virtual Wards, supporting admissions avoidance and early discharge from acute settings. Additionally, you will oversee various community health services, including MSK, diabetes service, district nursing, special schools, children with learning disabilities, SALT, physiotherapy, dietetics, podiatry, and TVN.

Main Duties of the Job:

The post holder will be responsible for:

Leading the development of the medicines management service for the Rapid Response Service Virtual Ward

Providing medicines management support and information to various community-based teams

Managing and delivering comprehensive prescribing advice and information

Ensuring safe, clinically effective, and cost-efficient use of medicines

Initiating, supervising, and undertaking medicine audits

Providing training and education for nursing and medical staff

Collating activity and outcome data

Actively developing and integrating policies, guidelines, and procedures

The applicant will be required to participate in the out-of-hours on-call service as well as the weekend morning service on a rota basis, with additional payments provided.
