Job description

Thisis an exciting opportunity to join the New Road Surgery team as a ClinicalPharmacist. The Clinical Pharmacist will join our pharmacy team,working with GPs and assisting them to provide high-quality, integrated patientcare.

Main duties of the job

The post holder will ideally be anexperienced pharmacist, who will act within their professional boundaries,supporting and working with the practices to ensure safe, effective and qualitymedicines management and quality care to patients.

NewRoad Surgery is a friendly, patient centred, training practice led by fourdoctors in partnership serving over 14,000 patients in Croxley Green, Sarrattand the surrounding area.

Job responsibilities

The post holder will undertake clinical medication reviews toproactively manage patients with complex polypharmacy, especially for olderpeople, people in residential care homes and those with multipleco-morbidities. The clinical pharmacist will be clinically managed by theprescribing leads at each practice.

The Clinical Pharmacist willwork as part of a multi-disciplinary team and part of the role will bepatient facing as required and decided by the individual practices.

The Clinical Pharmacist will provide primary support to generalpractice staff with regards to prescription and medication queries. They willhelp support the repeat prescription system, deal with acute prescriptionrequests, and medicines reconciliation on transfer of care and systems forsafer prescribing, providing expertise in clinical medicines advice whileaddressing both public and social care needs of patient in the GP practicesincluding care homes.

The Clinical Pharmacist willprovide clinical leadership on medicines management and optimisation, qualityimprovement and meeting guidelines and targets set by the local and directenhanced services and the quality outcome framework.
