We at the Grove Surgery have a new vacancy for a clinical pharmacist.

Main duties of the job

In this role, the post holder will work as an integral part of the practice team, contributing both administratively and in direct patient-facing interactions.

A key responsibility will be conducting clinical medication reviews to proactively manage patients with complex issues, ensuring their treatment regimens are both safe and effective.

The post holder will serve as a primary point of contact for general practice staff, providing expert advice on prescription and medication queries. They will play a crucial role in supporting the repeat prescription system, handling acute requests, conducting medicines reconciliation during transfers of care, and implementing systems for safer prescribing. Additionally, they will offer clinical guidance on medicines, addressing the public and social care needs of patients.

The post holder will take a leading role in medicines optimisation and quality improvement, overseeing aspects of the quality and outcomes framework as well as enhanced services.

Ensuring seamless integration between community pharmacies, the post holder will contribute to the efficient use of skill mix, enhancing patient outcomes, improving healthcare access, and helping to manage workload. This role is vital in driving improvements in both quality of care and operational efficiencies, requiring motivation and a passion for delivering exceptional service within general practice.

About us

Based at Netherfield in Milton Keynes with 3 GP partners, 3 nursing team members, and 1 HCA, we are looking for an experienced Clinical Pharmacist to join our team for 24 hours per week across three days to serve our patients. Our mission is to improve the quality of life of our community by taking care of their health needs.
