Job summary

5 Star Recruitment is seeking a Substance Use Recovery Worker to provide effective case management for service users with substance use issues. You will carry out comprehensive assessments, formulate risk management plans, and develop person-centred recovery plans in partnership with service users and their support networks.

You will deliver group interventions, maintain high-quality records, and work with multiagency partners to meet performance indicators.

Responsibilities

  • Provide effective case management of service users with substance use issues.

  • Deliver harm reduction interventions including needle exchange and health screening.
  • Ensure compliance with information governance and organisational policies.
  • Participate in meetings and represent the service with partner agencies.
  • Support staff through supervision, training, and personal development planning.
  • Provide high quality care while safeguarding service users from harm.

About the job PH & SU - Substance Use Recovery Worker

What I am accountable for:

Effective case management of service users with substance use issues

To carry out comprehensive substance use assessments including assessment of personal strengths and identification of any risk factors

Formulate, monitor and review risk assessments and risk management plans that effectively safeguard and protect service users and families/carers from harm

To develop person centred recovery plans using a strengths based approach in partnership with service users and others involved in their support network

Maintain a high standard of case notes and prepare and present narrative reports as required

Effective delivery of Group intervention sessions

Consistent, timely and accurate collection, recording and reporting of data and information relating to a client treatment journey

Ensuring achievement of key performance indicators relevant to your role

Effective multiagency working with statutory and nonstatutory agencies in relation to the wider presenting needs and complexities of service users

Deliver and support harm reduction interventions, including needle exchange and access to physical health care including BBV and health screening

Ensuring all paperwork and recording in accordance with our policy and procedure and follow information governance guidelines.

WHAT I NEED:

Substance use knowledge

Experience and knowledge of effective caseload management

Demonstrable skills and knowledge in assessing risk presented by clients to themselves and others

Knowledge of engagement and intervention strategies

Harm reduction, suicide and self-harm aware

Organised with IT skills; Ability to manage own time, prioritise activities and maintain accountability for your work

Experience of working in partnership with different agencies

Working as part of a team with colleagues including providing cover for planned and unplanned absences

Actively participate in multi-disciplinary team meetings and other service meetings

Thorough and timely communication with partner agencies e.g. GPs, Safeguarding, Criminal Justice Partners

Undertaking training to enhance skills, knowledge and practice

Seeking advice from colleagues, subject matter experts, clinical staff and managers to support practice

Actively engaging in your own supervision, Ongoing Personal Review and Personal Development Planning Process

Representing us and our organisational values at meetings and events; Ensuring an environment free from discrimination

Prioritising safeguarding of children and vulnerable adults and escalating any concerns.

Providing high quality case management to a caseload of service users including assessment, recovery planning, risk assessments and case reviews ensuring completion within organisational policy timescales

Delivery of psychosocial treatment interventions in groups and on a 1:2:1 basis

Supporting service users to access clinical interventions where required including engaging in clinical reviews, ensuring these occur within organisational policy

Providing prescription to service users where these are not posted to pharmacies

Distributing and training service users in naloxone

Delivering harm reduction advice including distributing clean injecting equipment through needle syringe programmes

Completing urine testing and dry blood spot testing following appropriate infection control procedures

Ensuring access to Recovery Support interventions including onward referrals

High quality record keeping for service users in treatment

Completing all tasks and documentation to ensure achievement of key performance indicators relevant to your role

Writing reports as required (e.g. safeguarding reports).
