Job description

We are pleased to invite applications to join the Breast Clinical Nurse Specialist (CNS) team based at Parapet, Windsor.

More people are living with and beyond cancer. Receiving care that is tailored to a person’s needs can have a significant impact on their experience and quality of life. Providing personalised stratified follow-up is a vital part of delivering world-class cancer services.

Supported by the Band 8a Cross-Site CNS Team lead, the successful post holder will work with cross-site colleagues to establish a dedicated team to advance the Trust’s Supported-Self-Management-Pathway (SSMP) service, also known as Patient Initiated Follow-Up (PIFU), aligning with national cancer strategies to deliver personalised care.

The CNS plays a pivotal role in ensuring an excellent care experience for breast cancer patients during and beyond their treatment. The post holder will also provide ad hoc support to the Primary CNS team, assisting patients newly diagnosed with primary breast cancer throughout their treatment. The post holder will gain experience in care coordination, service delivery during active treatment, and leading the transition to supported self-management.

The role is primarily based at The Parapet, Windsor, with a cross-site overview.

Main duties of the job
  • Establishing a dedicated team, responsible for developing and delivering a comprehensive, nurse-led service that is holistic, evidence-based, and tailored to individual needs of breast cancer patients post-treatment.
  • Working autonomously and within a team to develop quality improvement opportunities within personalised breast cancer care.
  • Coordinating care with the multidisciplinary team (MDT).
  • Delivering nurse-led clinics and providing specialist advice on breast awareness, including signs and symptoms of recurrence.
  • Facilitating rapid re-access to the breast cancer team via telephone advice and support for patients worried about symptoms or side effects.
  • Collaborating with the Macmillan Information and Support Service to develop initiatives such as Health and Well-Being Days.
  • Providing administrative support as needed.
