Personal Care Worker – Transitional Care (Hospital to Home)

At Right at Home Gold Coast, we specialise in delivering high-quality, compassionate care that allows people to remain safely and confidently at home. We provide personalised support services to older Australians, people recovering from surgery, and individuals transitioning from hospital stays back into their own homes.

We’re seeking Personal Care Workers to support our growing Transitional Care Program, helping clients regain independence after discharge from hospital or aged care settings.

About the Role

As a Personal Care Worker supporting transitional care clients, your role will focus on helping individuals adjust to life at home following medical treatment or rehabilitation. Working closely with family members, nurses, and allied health teams, you will ensure that care is delivered safely, respectfully, and with compassion.

Key responsibilities include:

Assisting with personal care (showering, grooming, toileting, mobility support)

Medication prompts and wellness checks

Supporting light household tasks such as laundry, meal prep, and cleaning

Providing safe transport to appointments or community activities

Communicating with care coordinators and family members to report progress

Monitoring client wellbeing and identifying risks during recovery

What You’ll Need

Certificate III or IV in Aged Care / Individual Support / Disability, or equivalent experience

Current First Aid and CPR certification

NDIS Worker Screening Check (or willingness to obtain)

NDIS Orientation Certificate

National Police Clearance

Australian Driver’s Licence + reliable, insured vehicle

100 Points of ID (e.g., Passport, Medicare Card, Birth Certificate)
