Job summary

4Recruitment Services are seeking to recruit a Personal Advisor (Leaving Care Services) for our client based in Liverpool. The role contributes to delivering a safeguarding and welfare focused children’s social care service, supporting care leavers to access education, housing, health and employment.

You will assess and monitor Pathway Plans, work with birth families where appropriate, coordinate services across agencies, and ensure data accuracy and confidentiality in line with statutory

Qualifications

  • Substantial experience of working with young people.
  • Experience with agencies and social care professionals in a multi-cultural context.
  • Experience handling sensitive and confidential matters under pressure.
  • Ability to formulate intervention and service plans for young people.
  • Full driving license and daily access to a vehicle.
  • Enhanced DBS check.

Responsibilities

  • Provide advice and practical support to care leavers on finances, accommodation, education, training, employment and leisure.
  • Participate in assessment and monitoring of Pathway Plans with a person-centred focus.
  • Promote contact with birth families and supervise as required.
  • Coordinate services to help care leavers access available support.
  • Maintain progress and well-being data for cases using appropriate methods.
  • Assist young people to access health services and build confidence and decision making.
  • Identify risks and report safeguarding issues as required.
  • Work with partners across council and external agencies.
  • Attend meetings and ensure timely, compliant data entry.

4Recruitment Services are seeking to recruit a Personal Advisor (Leaving Care Services) for our client based in Liverpool.

THE ROLE:

To contribute to the operational delivery of an effective Children's Social Care Service as part of a team responsible for safeguarding and promoting the welfare of children and young people who will be leaving or have left care provision, continuously improving outcomes.
To be responsible for support provision for allocated cases, in accordance with national and departmental procedures and guidance.
To contribute to managing the reputation of the service and the Council, complying with national and service procedures and guidance.

DUTIES AND RESPONSIBILITIES INCLUDE:
  • Provide advice and practical support to care leavers, including information and advice to enhance financial capability, accommodation options, education, training and employment, leisure and sporting and cultural opportunities to enable care leavers to enjoy and participate in community life.
  • Participate in the assessment, preparation, monitoring and review of Pathway Plans, ensuring a person-centred focus and empowering young people and their families.
  • Promote and facilitate contact for young people with their birth family members, providing supervision and assessment as required.
  • Co-ordinate the provision of services and take reasonable steps so that care leavers make use of services.
  • Maintain information about care leavers progress and well-being through a range of methods including direct contact.
  • Assist young people to maintain positive health and well-being as well as targeted advice and support to access specialist health services.
  • Support young people to develop their confidence and decision-making capacity.
  • Identify and assess levels of risk and need and take responsibility for reporting risk through the line management structure.
  • Identify and report safeguarding issues in line with legislation and local procedures.
  • Work co-operatively with colleagues and provide expertise/consultative service relating to young people's experiences in care and after they leave care.
  • Work in partnership within the Council and across agency boundaries including statutory, voluntary and local community and faith groups.
  • Attend and represent the department at a range of meetings including reviews and case conference meetings.
  • Be responsible, as specified in Departmental Guidance, for accurate, timely and up to date data entry on all cases including: data entry on IC and any other electronic tools or database, data required for specific PIs/targets, Ensure that client information data is lawfully gathered, accurate, up to date and only divulged in accordance with the Data Protection Act 1998 and the local government common law duty of confidentiality. Failure to apply these duties can lead to the individual or the Department facing court proceedings.
ESSENTIAL REQUIREMENTS INCLUDE:
  • Substantial experience of working and engaging with young people.
  • Experience of working with a variety of different agencies and social care professionals and working in a multi-racial/multi-cultural community.
  • Experience of dealing effectively with sensitive and confidential matters, pressure and conflict.
  • Ability to formulate plans for intervention and service provision for young people.
  • The role holder must hold a full driving license and have access to a vehicle on a daily basis.
  • Enhanced DBS check
