Job description

We have a new opportunity for a Permanent Band 6 Children’s Nurse Case Manager in South London.

Key Details:
  • Start Date: ASAP
  • Shift Days: Monday to Friday
  • Shift Hours: 09:00am – 17:00pm, with one evening per fortnight on call. Full flexibility considered.
  • Salary: Up to £45,000 per annum depending on experience, plus Oyster Card and mileage reimbursement.
Role Responsibilities:

As a Nurse Case Manager (Tracheostomy Experienced), you will:

  • Provide face-to-face support to clients and their families.
  • Create personalized care plans for each client.
  • Work closely with a core team of carers, training and supporting them with clinical skills and activities.
Client Conditions:

This role involves working with clients with conditions such as:

  • Spinal cord injuries
  • Neuromuscular disabilities
  • Tracheostomy / ventilation
  • Acquired brain injuries
  • Learning & physical disabilities
  • Epilepsy
Candidate Requirements:
  • Children’s community nursing experience, RSCN or RN Child Branch Diploma/Degree.
  • Evidence of relevant post-qualification experience and ongoing clinical training.
  • Experience caring for children with complex health needs, including tracheostomy care and long-term ventilation (oxygen therapy, suction, enteral feeds, catheterisation).
  • Knowledge of spinal injury care is desirable.
  • Enteral feeding experience is essential.
  • A qualified nurse will always be available for advice and support via on-call.
  • Experience caring for children with life-limiting or threatening illnesses.
