Job summary

An established industry player is seeking a dedicated support worker to assist participants in their recovery journey. This role involves providing one-on-one support, developing action plans, and collaborating with a team to deliver effective interventions for individuals facing mental health challenges or substance misuse.   • Provide support and guidance to project participants.
  • Develop ongoing action plans with participants for regular review.
  • Support team leader in raising service awareness.

Job description

Hours: 30 hours per week

Salary: £24,355 per annum (Pro Rata)

Based at: Dawn Centre, Conwy

Role Purpose
  • Provide support and guidance to all participants that have been registered onto the project.
  • To develop ongoing action plans with participants which will be reviewed on a regular basis.
  • To provide one to one support and group activities to aid the development of a participant in their recovery.
  • Work alongside the team structure in delivering services and outcomes for the scheme.
  • To achieve project targets and outcomes set by Welsh Government.
  • To support the team leader and referral agents in raising awareness of the service.
  • To work in partnership with other practitioners to deliver effective interventions and support for people with Mental Health and/or substance misuse.
  • Provide support and supervision to volunteers.
