We are looking for a Private Duty LPN in Highland, IL.

Pay Rate: $28.00/hr-$35.00/hr

Responsibilities:

  1. Assists in developing and revising the care plan.
  2. Provides nursing care per the physician’s treatment plan and client’s payer authorization.
  3. Monitors, records, and reports treatment reactions and changes in condition to relevant personnel.
  4. Provides ongoing instruction to clients, families, and healthcare team members.
  5. Records patient condition and care services in electronic charts.
  6. Participates in educational programs, quality assessments, and staff meetings.

Requirements:

  1. Active Licensed Practical Nurse or Registered Nurse License
  2. Problem-solving skills and ability to multi-task
  3. Excellent Communication Skills
