South Hardwick Primary Care Network is seeking to recruit an experienced, enthusiastic and motivated individual to join our expanding PCN team to work as a PCN and Home Visiting Service Administrator. The post holder will provide administrative support to all members of the PCN but mostly will work alongside the PCN Management team and our Home Visiting Service to provide care to our housebound patients and patients residing in care or residential homes.
Although we have advertised this post as Full or Part-time, the post-holder will be required to work Mondays to Wednesdays as a minimum.
This is an exciting opportunity to be at the forefront of the delivery of our Team Up/Ageing Well model of care, which provides acute and holistic care for the PCN's housebound and care home population. You will also play a critical role in the development of South Hardwick PCN, and its ambition to meet the NHS 10-year plan.
The post-holder must have excellent administrative, secretarial and communication skills to deliver high standards of work, and act in a professional and trustworthy manner as you will be the first contact for the PCN and Home Visiting service. You will need to be confident in working under pressure and be able to use your own initiative.
It will also be necessary to project a positive, friendly and empathetic image to patients, other services, and visitors, whilst developing and maintaining constructive relationships with internal and external stakeholders.
The post-holder will provide a full and comprehensive administrative service which supports all members of the PCN but mostly will work alongside the PCN Management Team and our Home Visiting Service.

£24,071 to £25,674 a year Pro rata depending on skills and experience
