About us

The Health Triangle PCN comprises 3 practices serving approximately 52,000 patients across Bracknell, Crowthorne, Owlsmoor, and Sandhurst. We are dedicated to delivering excellent primary care and have clinicians with expertise in cardiology, haematology, palliative care, immunology, dermatology, minor surgery, joint injections, LARCs, and frailty. We foster a strong ethos of support and education, offering protected educational and clinical meetings several times a month, and mentoring ST and FY doctors, medical students, nursing students, and pharmacy students. We are actively involved within our ICB and Federation.

Job responsibilities

We are a friendly, forward-thinking PCN covering Bracknell, Crowthorne, and Sandhurst. Our large GP and clinical team aim to deliver excellence and innovation in primary care.

Our ethos emphasizes education and support, with each salaried GP having a GP Partner as a mentor. We hold educational and clinical meetings multiple times a month, including a full afternoon per month dedicated to protected teaching sessions. We also train GP STs and FY doctors, medical students, nursing students, pharmacy students, and physician associate students.

We are highly active within our ICS and Federation, with clinical lead positions within the ICB. The PCN has delivered innovative projects, including a Cancer Quality Care Project and a Dermatology Drive-Through Service, which has received national recognition.
