Qualifications

Job description

An exciting opportunity is available now to work as a Clinical Pharmacist within the Clayton, Beswick and Openshaw Primary Care Network - a forward-thinking and innovative PCN made up of 7 GP practices serving a total population of 47,592 patients.

The successful candidate(s) will be based within the GP practices and may be required to work across different practices, with some remote/home working as part of the role.

Our workforce is growing rapidly, and these roles will embed into a multidisciplinary team of GPs, ANPs, pharmacists, pharmacy technicians, practice nurses, mental health practitioners, social prescribers, and physiotherapists to provide optimal care for our community in North Manchester.

Main duties of the job

You will work as part of a multidisciplinary team to develop and run processes for repeat prescription re-authorisation, management of medicines on transfer of care, systems for safer prescribing, clinical audits, Standard Medication Reviews, and caseload management of patients with long-term conditions. You will also support work in local Care Homes, looking after these vulnerable patients.

The post holder will work within their clinical competencies as part of a multidisciplinary team to provide expertise in clinical medicines management, conduct face-to-face structured medication reviews, manage long-term conditions, oversee medicines transfer of care, and implement systems for safer prescribing. Responsibilities also include managing repeat prescriptions, acute requests, and addressing public health and social care needs.

The role involves face-to-face medication reviews, especially for patients with polypharmacy, older adults, and those in residential care or with multiple co-morbidities. You will provide leadership on quality improvement, clinical audit, and manage aspects of the Quality and Outcomes Framework. The role is pivotal in improving care quality and operational efficiency, requiring motivation and a passion to deliver excellent service.

You will have the opportunity to develop into a non-medical prescriber if not already qualified.

Key duties include:

  • Managing medicine optimisation for patients with single or multiple medical problems.
  • Conducting clinical medication reviews with patients, care home staff, and in domiciliary settings.
  • Identifying high-risk patients through data searches and reducing hospital admissions related to medicines.
  • Managing common and minor ailments within scope, signposting appropriately.
  • Reconciling medicines post-hospital discharge and ensuring continuity of supply.
  • Providing medicine information and advice to practice staff and patients.
  • Signposting patients to appropriate healthcare professionals.
  • Managing repeat prescribing processes and monitoring.
  • Analyzing medicines data, conducting audits, and implementing changes based on guidance and alerts.
  • Monitoring prescribing against guidelines and formulary, providing updates and education.
  • Supporting practice compliance with CQC standards related to medicines.
  • Supporting public health campaigns and collaborative working within the PCN MDT.
Person Specification
Experience
  • Minimum of two years' experience as a pharmacist, ideally in hospital, general practice, or CCG.
  • Experience working in primary care with other healthcare professionals.
  • Experience in the care home sector and with CCGs.
  • Experience providing evaluated drug information.
Qualifications
  • Registered member of the General Pharmaceutical Council.
  • Recent CPD or professional development evidence.
  • Post-graduate clinical qualification, diploma level or higher.
  • Completion of CPPE 18-month Primary Care Pharmacy pathway.
  • Prescribing qualification.
