An exciting opportunity exists for a Clinical Pharmacist to join the Clayton, Beswick, and Openshaw Primary Care Network in Manchester. You will work with a multidisciplinary team to enhance medication management and improve patient care. An exciting opportunity is available now to work as a Clinical Pharmacist within the Clayton, Beswick and Openshaw Primary Care Network - a forward-thinking and innovative PCN made up of 7 GP practices serving a total population of 47,592 patients.
The successful candidate(s) will be based within the GP practices and may be required to work across different practices, with some remote/home working as part of the role.
Our workforce is growing rapidly, and these roles will embed into a multidisciplinary team of GPs, ANPs, pharmacists, pharmacy technicians, practice nurses, mental health practitioners, social prescribers, and physiotherapists to provide optimal care for our community in North Manchester.

You will work as part of a multidisciplinary team to develop and run processes for repeat prescription re-authorisation, management of medicines on transfer of care, systems for safer prescribing, clinical audits, Standard Medication Reviews, and caseload management of patients with long-term conditions. You will also support work in local Care Homes, looking after these vulnerable patients.

Job responsibilities
The post holder will work within their clinical competencies as part of a multidisciplinary team to provide expertise in clinical medicines management, conduct face-to-face structured medication reviews, manage long-term conditions, oversee medicines transfer of care, and implement systems for safer prescribing. Responsibilities also include managing repeat prescriptions, acute requests, and addressing public health and social care needs.
The role involves face-to-face medication reviews, especially for patients with polypharmacy, older adults, and those in residential care or with multiple co-morbidities. You will provide leadership on quality improvement, clinical audit, and manage aspects of the Quality and Outcomes Framework. The role is pivotal in improving care quality and operational efficiency, requiring motivation and a passion to deliver excellent service.
You will have the opportunity to develop into a non-medical prescriber if not already qualified.
