Job description

An exciting opportunity is available now to work as a Clinical Pharmacist within the Clayton, Beswick and Openshaw Primary Care Network - a forward-thinking and innovative PCN made up of 7 GP practices serving a total population of 47,592 patients.

The successful candidate(s) will be based within the GP practices and may be required to work across different practices, with some remote/home working as part of the role.

Our workforce is growing rapidly, and these roles will embed into a multi-disciplinary team of GP's, ANPs, pharmacists, pharmacy technicians, practice nurses, mental health practitioners, social prescribers, and physiotherapists, providing optimal care for our community in North Manchester.

Previous experience in primary care is desirable.

Main duties of the job

You will work as part of a multi-disciplinary team to develop and run processes for repeat prescription re-authorisation, management of medicines on transfer of care, systems for safer prescribing, clinical audits, Standard Medication Reviews, and caseload management of patients with long-term conditions. You will also support work in local Care Homes, focusing on vulnerable patients.

The post holder will work within their clinical competencies as part of a multi-disciplinary team to provide expertise in clinical medicines management, conduct face-to-face structured medication reviews, manage long-term conditions, oversee medicines transfer and safety systems, manage repeat prescriptions, and address public health and social care needs.

Responsibilities include medication reviews for patients with polypharmacy, especially in older adults and care homes; leadership in quality improvement and clinical audit; supporting development as a non-medical prescriber; and various clinical and administrative tasks related to medicines management and prescribing.

Further duties involve reviewing high-risk medication cohorts, managing medicines at discharge, providing medication information, signposting patients, managing repeat prescribing, analyzing medicines data, implementing guidelines, providing education, ensuring CQC compliance, supporting public health campaigns, and collaborating with PCN MDT and external colleagues.

Person Specification
Experience
  • Minimum of two years experience as a pharmacist, with prior experience in hospital, general practice, or CCG settings.
  • Experience working collaboratively with healthcare professionals in primary care.
  • Experience in the care home sector and with CCGs.
  • Experience providing evaluated drug information.
Qualifications
  • Registered member of the General Pharmaceutical Council.
  • Evidence of recent CPD or professional development.
  • Post-graduate clinical qualification, diploma level or higher.
  • Completion of CPPE 18-month Primary Care Pharmacy pathway.
  • Prescribing qualification.
