Job description

A new opportunity has arisen for a PCN Operations Lead to join our team as part of the Picton Primary Care Network (PCN)NHS. This is a hugely exciting opportunity to work innovatively across ournetwork to support the growth of our support services within the community.

The PCNOperations Lead will provide day-to-day operational support to the CareCoordinator team within the Primary Care Network (PCN). Working under thedirection of the PCN Manager, they will ensure high-quality,patient-centred delivery of PCN services, effective team operations, andalignment with PCN priorities including practice, personalised care.

Main duties of the job

The postholder will lead and manage the Care Coordinator team, providing day-to-day supervision, performance management, and support with recruitment, induction, and training. They will be responsible for team rotas, workload allocation, and monitoring KPIs to ensure effective and high-quality service delivery. A key focus will be on fostering a positive, inclusive team culture that promotes wellbeing and continuous learning.

They will oversee the delivery of care coordination services aligned with national and local priorities, including proactive care for complex households, support for care homes, social prescribing and personalised care.

They will be required to communicate effectively and sensitively to patients, clinicians and practice admin staff across the Network GP practices.

Our Care Coordinator team provides support across a widerange of areas, including housebound care, cancer screening, mental health,womens health, and chronic disease recall. They play a key role in ensuringpatients receive proactive, personalised care by helping to coordinate appointments,follow-ups, and targeted health interventions. In addition to their day-to-daycoordination work, the team also leads on patient engagement and outreachevents within the community, helping to reduce health inequalities and improveoutcomes across the population.

Job responsibilities

Team Leadership and Line Management

  • Linemanage the Care Coordinator team, providing day-to-day oversight and supervision, as well as supporting appraisals, and performance management.
  • Supportrecruitment, induction, and training of new care coordinator staff.
  • Developteam rotas, manage workloads, and monitor KPIs to ensure effective servicedelivery.
  • Createa positive, inclusive team culture that promotes learning, wellbeing, and highstandards.

Service Delivery and Operational Oversight

  • Ensurethe Care Coordinator team delivers against local and national servicerequirements, including proactive care for complex patients, enhanced health incare home and social prescribing and personalised care initiatives.
  • Monitorquality, efficiency, and patient outcomes, making service improvements asneeded.
  • Actas a first point of escalation for operational issues within the PCN.

Support to PCN Management

  • Workwith the PCN Manager to implement PCN objectives related to care coordinationand wider workforce development.
  • Contributeto service development plans, workforce planning, and reporting to the ICB orother partners.
  • Providedata and insights on performance, capacity, and outcomes to support serviceevaluations.

Stakeholder Engagement

  • Collaboratewith general practice teams, social prescribers, health coaches, communityteams, and voluntary sector partners.

Governance and Compliance

  • Ensurethat the team adheres to all relevant information governance, safeguarding, andhealth and safety policies.
  • Maintainup-to-date records, including the use of digital tools and patient recordsystems (e.g. EMIS).
  • Supportaudits, risk assessments, and service reviews.

Qualifications
  • Educated to degree level or equivalent experience.
  • Qualification or training in leadership or project management.
