An Integrated Care System is seeking an Experienced General Practice Nurse for their care home team in Lytham. The role involves managing long-term conditions and ensuring quality health care for a diverse patient population. With responsibilities that include mentoring staff and conducting essential health checks, this position is pivotal in enhancing patient care within the community.
LythamSt Annes PCN are looking to recruit an Experienced General Practice Nursewithin the ARRS roles to join our care home team, to help us to transform andmodernise pathways of care, enabling the safe and effective sharing of skillsacross traditional professional boundaries. The post holder will help reduce GPservice demand by conducting Care home visits. There are approximately 35 carehomes consisting of nursing and residential beds. The care home population isapproximately 900. Residents often have multiple comorbidities/Chronic diseasesthat require regular monitoring and reviews.
The post holder will workto integrate practices and community services to help utilise skill mix,improve patient outcomes, ensure better access to healthcare and help manageworkload. The role is pivotal to improving access to treatment. The post holderwill need to work flexibly to meet the needs of the surgeries within theprimary care network.

Capable of managing Long Term Conditions
A Primary Care Network (PCN) enablesthe provision of proactive, accessible, coordinatedand integrated primary and community care, improving outcomes forpatients. They are formed around naturalcommunities based on GP registered lists, serving populations of around 30,000to 50,000.
The Lytham St Annes Primary Care Network includes, 5GP Practices, Holland House, Fernbank, Parcliffe, Poplar and Ansdell, servinga population of 53,772 registered patients.

Capable ofmanaging Long Term Conditions
Long-Term Condition Management & Elderly Care
Leadand manage long-term conditions such as hypertension, diabetes, asthma, COPD,and cholesterol management.
Providecare for those patients living in Care homes.
Supportquality improvement and assurance initiatives within the PCN.
GeneralPractice Nursing Duties within a Care Home Setting
Conductchronic disease management within the scope of expertise.
Provideimmunisations, including:
oRoutineinfluenza, pneumococcal, shingles vaccinations and COVID boosters
Carryout ECGs, venepuncture, urinalysis, and wound care (including ulcer managementand suture removal).
Reviewand action pathology tests such as urine culture and swabs where appropriate.
Chaperoneand assist patients being examined by other clinicians.
Leadership & Team Support
Mentorand supervise new-to-practice nurses, nursing associates, and HCAs.
Workcollaboratively with the wider practice team to enhance patient care.
Promotepublic health and screening programs
Integratepopulation health management approaches to reduce health inequalities.
Clinical Governance & Compliance
Maintainaccurate and up-to-date patient records.
Ensurecompliance with clinical protocols, including prescribing audits and reviews ifqualified.
Adhereto NMC revalidation requirements and practice within the NMC Code of Conduct.
Followinfection control policies and ensure safe handling of specimens.
Contributeto audits and service evaluations.
Supportthe effective use of NHS resources and practice policies.
Ensurethe cleanliness and tidiness of nursing and treatment rooms.
Maintainand clean clinical equipment.
Monitorand order medical stock, drugs, and vaccinations.
Attendand contribute to practice meetings and training sessions.
Health & Safety Responsibilities
Complywith national infection control standards and health and safety guidelines.
Ensuresafe handling, storage, and transport of specimens.
Identifyrisks in work activities and take necessary precautions.
