Mosaic Primary Care Network consists of 3 practices (Crown Heights Medical Centre, Odiham & Old Basing Health Centres and Bramblys Grange Medical Practice) located in or near Basingstoke. We have a combined patient population of circa 60,000 patients.

We are seeking a compassionate and proactive Healthcare Assistant (HCA) to join our friendly General Practice team at Odiham and Old Basing Health Centres. The HCA will support the clinical team in delivering high-quality care to patients, performing a variety of duties including clinical procedures, administrative tasks, and assisting with health promotion and chronic disease management

Main duties of the job

Key Responsibilities:

Clinical Duties:

Perform routine clinical tasks under supervision such as:

Phlebotomy (blood taking)

Blood pressure monitoring

ECG recording

Urinalysis

Height, weight, and BMI measurements

NHS Health Checks

Wound care and dressings (if trained)

Flu and covid vaccinations (once trained)

Assist GPs and nurses during minor procedures or womens health clinics

Support chronic disease monitoring (e.g. diabetes, asthma) in collaboration with nursing staff

Liaise with wider MDT for continuity of patient care

Support continuity of care and patient safety initiatives

Support health promotion and prevention activities i

Tasks will be completed under the supervision of a registered clinician while developing competencies in primary care.

Administrative Duties:

Record clinical observations accurately in patient records (EMIS)

Ensure medical equipment is clean and stocked

Assist with stock control and ordering of clinical supplies

Support data collection for audits and quality initiatives (e.g., QOF, PCN DES)

Manage waiting list recall ie, Spirometry, coils, implants, dopplers

You will need to work from both Odiham and Old Basing Health Centres

You must have completed the care certificate or equivalent

You will need to hold a full driving license and have access to your own transport

Qualifications and Experience:

Has completed the Care Certificate or equivalent qualification

Is working at Support Worker Level Practice as described in the Primary Care and General Practice Nursing Career and Core Capabilities Framework

Experience working as a Healthcare Assistant in a primary care or community setting

Phlebotomy training and recent experience

Relevant experience in health promotion and prevention and supporting long-term condition management

Good communication and interpersonal skills

Ability to work both independently and as part of a multidisciplinary team

Proficient in IT systems (e.g. Microsoft Office)

IM medication administration

Proficient in using EMIS
