Job summary

Primary Care Dietitians Ltd in England is seeking an experienced dietitian to join a PCN team, delivering specialist dietetic services directly within GP practices.

You will manage a caseload, support patients with pre-diabetes, type 2 diabetes, weight management and frailty, and contribute to service improvement, audits and pathway development; group education may form part of the role.

• Support patients with pre-diabetes, type 2 diabetes, weight management and frailty.
• Provide person-centred care and behaviour change approaches.
• Contribute to service improvement, clinical audit and pathway development.
• Group education sessions may form part of the role depending on service needs.

Job description

We are looking for an experienced dietitian to work within a Primary Care Network in Haverings. This is an exciting opportunity to work at the forefront of primary care, delivering specialist dietetic services directly within GP practices. As a PCN Dietitian, you will play a vital role in supporting patients with nutrition-related conditions, helping to reduce pressure on GPs while improving access to high-quality dietetic care in the community., You will manage your own clinical caseload, working autonomously within GP practices as part of a wider multidisciplinary team. Your focus will be supporting patients with conditions such as pre-diabetes, type 2 diabetes, weight management, and frailty - helping them make lasting changes through person-centred care and behaviour change approaches. You will also have opportunities to contribute to service improvement, clinical audit, and pathway development. Group education sessions may form part of the role depending on service needs.
