An East Midlands-based Primary Care Network is seeking an enthusiastic Registered General Nurse to provide high quality community nursing to housebound patients. The role involves managing long-term conditions, carrying out immunisations, and collaborating with GP practices to ensure seamless care. This fixed-term contract covers maternity leave and offers a supportive environment with opportunities for professional development.
Employed through PICS but working for Mansfield North, we have an exciting opportunity for an RGN (fixed term contract to cover maternity leave) to join the PCN to support the practices in the delivery of Primary Care nursing to housebound patients. The role will be based in the community, delivering nursing care on behalf of the GP practices to patients in their own home including care homes. There may be the requirement to occasionally deliver care within the GP practices and support PCN pilots and projects as required.
We are looking for an RGN who is enthusiastic about providing high quality care. The successful candidate will be able to undertake annual reviews for long-term conditions, adult immunisations and other clinical procedures. They will need to have a good knowledge of the Quality and Outcome Framework and frailty. A non-medical prescribing qualification would be advantageous but not essential.
The postholder will need to be quick to learn, hardworking, highly motivated and have the confidence to work independently as well as part of a team. They will need to be able to support junior colleagues in the team.
The postholder will be supported by the existing Community Practice Nurse and the PICS Lead for PCN nursing.

KEY RESPONSIBILITIES

To work closely with nursing and administrative colleagues in the team
To liaise with GP practice colleagues to ensure seamless care between the community team and GP practice

KEY RESPONSIBILITIES
Clinical Practice:
To assess, design, implement and evaluate safe, creative and effective treatment plans in collaboration with the patients diverse needs, their carers and other professionals.
Responsible for the safe monitoring, management and administration of medication as required by PICS policy.
To maintain safeguarding responsibilities for children and vulnerable adults in line with PICS policy, and Local Authority procedure and guidance, including making timely referrals as required.
To maintain accurate, timely and relevant clinical records both written and computerised.
To be aware of and apply principles on infection prevention and control and to comply with policy, procedure and guidance.
To provide integrated care following a holistic assessment which enables patients to be cared for in their own homes and clinic settings. The post holder will facilitate the prevention of unnecessary hospital admission/re-admission by reducing the effects of ill health and disability and maximising the individuals potential for independence.
To manage patients living with long-term conditions, including frailty, and provide relevant care as necessary. This may include non-medical prescribing.
Immunisation:
Support the PCN GP Practices by carrying out immunisations within patient homes, including care homes.
Undertake opportunistic and domiciliary immunisations, following up on non-immunised patients; providing information, advice and updating dates as required.
Provide outreach contact to access care needs of patients as delegated by the practices.
Be responsible for carrying out a routine enhanced immunisation programme.
Collate, collect and monitor data to support immunisation statistical returns.
Work collaboratively with team members and partners to meet immunisation targets for the practice.
Communication:
Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment.
Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating.
Anticipate barriers to communication and take action to improve communication.
Estimate and maintain communication with individuals and groups within the PCN and external stakeholders.
Act as an advocate when representing the patients and colleagues viewpoints to others.
Delivery Quality Service:
Recognise and work within own competence and professional code of conduct as regulated by the Nursing and Midwifery Council (NMC 2018).
Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures.
Prioritise, organise and manage own workload in a manner that maintains and promotes quality.
Deliver care according to National Institute for Clinical Excellence (NICE) guidelines, local guidelines and evidence-based care.
Assess effectiveness of care delivery through self and peer review, benchmarking and formal evaluation.
Participate in the maintenance of quality governance systems and processes across the organisation and its activities.
Utilise the audit cycle as a means of evaluating the quality of the work of self and the team, implementing improvements where required.
In partnership with other clinical teams, collaborate on improving the quality of health care, responding to local and national policies and initiatives as appropriate.
Evaluate the patients response to health care provision and the effectiveness of care.
Support and participate in shared learning across the practice and wider organisation.
Participate in the management, review and identify learning from patient complaints, clinical incidents and near-miss events utilising a structured framework.
Participate in the performance monitoring review of the team, providing feedback as appropriate.
Understand and apply legal policy that supports the identification of vulnerable and abused children and adults, being aware of statutory child/vulnerable adult health procedure and local guidance.
Work within policies relating to domestic violence, vulnerable adults, substance abuse and addictive behaviour, and refer as appropriate.
Team Working:
Understand own role and scope in the organisation and identify how this may develop over time.
Work as an effective and responsible team member, supporting others and exploring the mechanisms to develop new ways of working.
Delegate clearly and appropriately, adopting the principles of safe practice and assessment of competence of those taking on delegated duties.

Management of Risk:
Manage and assess risk within the areas of responsibility, ensuring adequate measures are in place to protect staff and patients.
Monitor work areas to ensure they are safe and free from hazards and conform to health, safety and security legislation, policies, procedures and guidelines.
Ensure safe storage, rotation and disposal of vaccines and drugs is undertaken. Where appropriate, oversee the monitoring, stock control and documentation of controlled drug usage according to legal requirements.
Undertake mandatory and statutory training and ensure that this is up to date.
Apply infection control measures within the practice according to practice, local and national guidelines.
Apply policies that reduce environmental health risks, are culturally sensitive and increase access to health care for all.
Participate in the local implementation strategies that are aligned to the values and culture of general practice.
Use technology as an aid to management in planning, implementation and monitoring, presenting and communicating information.
Review and process data using accurate Read codes about patients in order to ensure easy and accurate retrieval for monitoring and audit processes.
Manage information searches using the internet and local library databases, for example, the retrieval of relevant information for patients on their condition.
Understand own and others responsibility to the individual organisation regarding the Freedom of Information Act.
Learning and Development:
