Job summary

South Rural PCN seeks a PCN Clinical Pharmacist to join a multi-site team. The role supports hospitals and practices, optimising medication safety and patient care. Part-time or full-time, it covers Holbrook and Shotley, with activities across the PCN and network.

The successful candidate will be GPhC registered, with independent prescriber status or progression toward it, and experience in primary care pharmacy. NHS career benefits apply.

Qualifications

• GPhC registered pharmacist with UK registration.
• Independent prescriber qualification or progress toward one.
• Experience with CPPE primary care education pathway or equivalent.

Responsibilities

• Clinically assess and treat patients using medicines knowledge.
• Act as PCN point of contact for medicine-related matters.
• Standardise medicines management across the PCN where possible.
• Provide medicine reviews including domiciliary visits to nursing homes.
• Deliver long-term conditions clinics and manage complex medication regimes.
