Job description
Overview

IP Qualified Clinical Pharmacist specialising in Diabetes, based with North Connect PCN at Eastbury Surgery. You will work across several practices, developing this role with support from GPs and under a delegated clinical supervisor. The role suits a forward-thinking, innovative clinician who collaborates across the team.

Responsibilities
  • Patient facing Clinical Medication Review: undertake reviews within the practice, care homes and domiciliary visits to proactively manage polypharmacy and provide recommendations to senior clinical pharmacists, nurses and/or GPs.
  • Patient facing Long-term condition clinics: manage medicine optimisation for patients with single or multiple conditions (e.g., Respiratory, Cardiovascular, Diabetes) via face-to-face, telephone, home visits or video consultations; de-prescribe where appropriate and lead medicines optimisation and quality improvement.
  • Care Home and Domiciliary Clinical Medication Reviews: review and recommend to clinicians; improve safety of medicines ordering and administration with care home staff.
  • Manage common/minor/self-limiting ailments within scope of practice; signpost to community pharmacy and refer to GPs or other professionals when appropriate.
  • Medicines support: provide patient facing clinics and a telephone helpline for questions about medicines.
  • Medicine information to practice staff and patients: answer enquiries, suggest solutions, follow up on effects of changes, provide telephone support.
  • Reduce unplanned hospital admissions: review medicines linked to admissions and readmissions; implement changes to reduce prescribing to high-risk groups.
  • Medicines management at discharge: reconcile medicines after hospital discharge and ensure continuity of supply for high-risk groups.
  • Signposting: ensure appropriate referrals to healthcare professionals within appropriate timeframes.
  • Repeat prescribing: develop and implement a repeat prescribing policy; manage reauthorisation and monitoring requirements.
  • Risk stratification: identify high-risk patient cohorts via practice computer searches.
  • Service development and medicines quality improvement: provide pharmaceutical input for new services and conduct prescribing audits with GP leadership.
  • Medicines safety: implement changes from MHRA alerts and NICE guidance; audit compliance.
  • Education and Training: provide therapeutic education to primary care teams.
  • Care Quality Commission: support practice compliance with CQC standards where medicines are involved.
  • Collaborative working relationships: participate in the PCN multidisciplinary team; liaise with CCG colleagues; foster links across PCN and wider networks; recognise roles and communicate effectively with stakeholders.
  • Professional development: engage in CPD, supervision, and personal development plans; support formal education programs and keep clinical practice up to date.
  • Knowledge, Skills and Experience: evidence of pharmacy degree, GPhC registration, minimum 2 years experience; experience with common acute and long-term conditions; independent prescribing qualification in progress or held (not mandatory). Ability to follow policies and involve patients in decisions.

Special Working Conditions

Travel independently between practice sites (where applicable) and attend meetings hosted by other agencies as required.
