PCN Clinical Pharmacist - Hackney Downs PCN

The Hackney Downs PCN in City and Hackney is seeking to recruit an experienced PCN Clinical Pharmacist to develop and support medicines optimisation work within the GP practices in the PCN covering a population of approximately 40,000.
Main duties of the job
The post holder will work as part of a multi-disciplinary team in a patient-facing role. The PCN Clinical Pharmacist will take responsibility for areas of chronic disease management and work proactively with other healthcare professionals as part of a multidisciplinary team across the PCN.
The successful candidate:
- will join a core Pharmacy Team supported by a Senior Clinical Pharmacist and a GP Supervisor
- will be covering only 2 practices located in the same building/site, so no travel between surgeries needed
- entitled to re-imbursement of basic annual indemnity insurance
- will have another pharmacist with experience already working in both practices to support
- will be welcomed to do the independent prescribing course with us (if already finished CPPE/nearing the end), regular development sessions, teaching, training, and upskilling opportunities will be provided
- will have support via monthly group meetings with a senior pharmacist
We have a well-structured pharmacy team in Hackney Downs currently comprising 6 pharmacists and 2 pharmacy technicians across the PCN.
Clinical Pharmacists play a key role in supporting delivery of the new network contract Directly Enhanced Service (DES) specifications, including Extended Access.
About us
Groups of GP Practices in City and Hackney have formed 8 Primary Care Networks (PCNs), each covering a population of 30,000-56,000. These PCNs aim to enable GP practices to collaborate and develop network-based services tailored to local needs.
Job responsibilities
The PCN Clinical Pharmacist will:
- work as part of a multidisciplinary team in a patient-facing role, assessing and treating patients using their medicines expertise to support management of acute and long-term conditions
- be a prescriber or be training to become one, working alongside the general practice team
- manage patients with long-term conditions, undertake clinical medication reviews, especially for those with polypharmacy, elderly, or with multiple co-morbidities (e.g., frailty, COPD, asthma, diabetes, cardiovascular disease), including those with learning disabilities or autism (via STOMP program)
- provide specialist expertise in medicines use, addressing public health and social care needs, and tackling inequalities
- lead medicines optimisation systems, processes, and quality improvement initiatives
- contribute to meeting QoF targets, enhanced services, and IIF indicators
- support structured medication reviews to improve patient understanding, adherence, and self-care
- support integration of general practice with wider healthcare teams, including community and hospital pharmacy, to improve outcomes and access
- build relationships with pharmacy professionals across PCNs and the wider health and social care system
- manage shared care protocols, clinical research, and liaise with specialist pharmacists, including mental health and antipsychotic reduction programs
Key aspects of the pharmacist role
- conduct face-to-face medication reviews for patients with long-term conditions, polypharmacy, housebound patients, and other priority areas
- review ongoing medication needs, monitor requirements, and support safe, appropriate, and optimized treatments
- discuss recommendations with the PCN team and ensure timely follow-up
- support medication adherence through patient information leaflets and liaising with community pharmacists
- assist practices in improving access for minor ailments and acute presentations
- promote self-care and signpost to community pharmacy services
Medicines Information to Practice Staff and Patients
- resolve medicines queries from the practice team, PCN team, community pharmacists, and patients
- provide updates on medicines optimisation during clinical meetings
- analyze prescribing data and audit findings to highlight issues
- communicate safety alerts and manage medication reconciliation post-discharge
Commissioned improvements for QIPP and medicines optimisation
Support the practice and PCN in achieving outcomes related to medicines in the Clinical Commissioning Contract.
Signposting
Be aware of available services to support patients and refer appropriately, e.g., social prescriber, specialist referral.
Repeat Prescribing
- Review and support adherence to the practice’s repeat prescribing policy
- Manage re-authorization requests, review patient requests, and ensure monitoring is up-to-date
Clinical Governance
- Implement and support MHRA alerts, product withdrawals, and guidelines
- Conduct clinical audits and feedback results
- Report medicines incidents and support system improvements
- Support shared care protocols and monitor high-risk drugs
- Review medicines associated with hospital admissions and high-risk cohorts
Education and Training
Contribute to education on medicines optimisation for healthcare professionals and wider networks.
Public Health
Support public health campaigns and provide advice on public health programs.
General
Undertake other medicines optimisation work as agreed with management.
Key Collaborative Working Relationships
Work with a range of stakeholders, including patients, practice staff, healthcare professionals, and community teams, to support person-centered care and social prescribing.
Knowledge, Skills, and Experience
- Experience/awareness of common acute and long-term conditions in general practice
- Minimum 2 years postgraduate pharmacy experience
- Hold or be working towards independent prescribing qualification
- Understanding of the clinical pharmacist role within primary care
