Brackley and Towcester PCN Ltd is seeking an experienced Clinical Pharmacist to join our dynamic healthcare team at Towcester Medical Centre. This is an exciting opportunity to make a real difference in primary care, working closely with GPs, nurses, and other healthcare professionals to optimize patient medication management and outcomes.

Main duties of the job

Registered Pharmacist with the General Pharmaceutical Council (GPhC).

Must have completed an approved 18-month training pathway (such as CPPE Clinical Pharmacist pathways) that equips you to practice safely in primary care.

CPPE Primary Care Pharmacy Pathway completion is mandatory (requirement under Additional Roles Reimbursement Scheme).

Clinical experience in a healthcare setting.

Strong communication and interpersonal skills.

Ability to work independently and as part of a multidisciplinary team.

Commitment to continuing professional development and clinical supervision.

Computer literacy and familiarity with clinical systems.

Previous experience working in general practice or primary care.

Experience with clinical medication reviews and chronic disease management.

Knowledge of local prescribing guidelines, formularies, and antimicrobial stewardship.

Prescribing and Medication Management
  • Act as an independent prescriber (if qualified) working with and alongside the general practice team.
  • Prescribe medicines within scope of practice and competence (qualified prescribers only).
  • Work collaboratively with GPs and practice team to support medication decisions.
  • Provide clinical recommendations for medication therapy.
  • Review and advise on medication regimens and therapeutic options.
  • Ensure prescribing practices and recommendations align with local guidelines and formularies.
Chronic Disease Management
  • Manage patients with chronic conditions requiring complex medication regimens.

    • Complex polypharmacy cases

    • Patients with multiple co-morbidities (frailty, COPD, asthma)
    • People with learning disabilities or autism (STOMP programme)
  • Monitor treatment outcomes and adjust therapy as appropriate.
Medicines Optimisation
  • Lead medicines optimisation initiatives across the practice.
  • Provide specialist expertise in safe and effective medicines use.
  • Address medication-related health inequalities.
  • Support patients in understanding their medications and improving adherence.
  • Reduce medication waste through appropriate review and deprescribing.
Quality Improvement and Leadership
  • Lead quality improvement projects related to pharmaceutical care.
  • Contribute to Quality and Outcomes Framework (QOF) achievements.
  • Support enhanced services delivery.
  • Implement antimicrobial stewardship programmes.
  • Develop and maintain clinical protocols and guidelines.
Professional Collaboration and Integration

Person Specification
Experience
  • Experience of undertaking medication or medicine use reviews.
  • Experience of working with clinicians and multidisciplinary/multi-agency work.
  • Experience in undertaking clinical audit.
  • At least 2 years post-registration experience in a hospital, community or general practice setting.
Qualifications
  • Masters Degree in Pharmacy or equivalent.
  • Registered with GPhC.
  • Specialist knowledge through a Postgraduate Diploma & Qualification.
  • Evidence of recent CPD.
  • Independent Prescriber status (not essential).
