A healthcare organization in Goole is seeking a Clinical Pharmacist to enhance primary care services. You will manage chronic disease care, perform clinical medication reviews, and train healthcare teams. The ideal candidate holds a pharmacy degree, has at least two years of experience, and is dedicated to optimizing patient care in a multidisciplinary team.
We are looking torecruit a suitably qualified and registered Clinical Pharmacist under NHSEnglands Additional Roles Reimbursement Scheme (ARRS) to work in our expandingmultidisciplinary team (MDT) on behalf of Cygnet Primary Care Network memberpractices. We are hoping to recruit a Pharmacist working from one of our member practices.

The roles will bebased at one of the Goole PCN sites - Bartholomew Medial Practice.
You will be responsible for the care management ofpatients with chronic diseases and/or recent hospital admission plus undertakeclinical medication reviews to proactively manage people withcomplex polypharmacy especially those in care homes, the frail, and thosewith co-morbidities. You will be involved with shared decision making whichwill include involvement in regular care home rounds, prioritising patients forMDT review and detailed Care Plans. You will offer support to the Single Point of Access team that supports all the Care Homes in the network.
CygnetPCN covers a large geographical area within the East Riding of Yorkshire. Ournetwork covers over 52,000 patients and is made up of the five following GPPractices:

PCN's build on existing primary care services and enable greaterprovision of proactive, personalised, coordinated and more integrated healthand social care for people close to home. Our PCN are proactively providingcare and services for the people and communities we serve.
Newroles are being introduced to the network as we are expanding into differentareas of Healthcare.
Thenetwork provides a single point of access for the nursing, residential andlearning disability homes in the area.

Working within yourclinical competencies alongside the general practice teams you will prescribe,be responsible for the care management of patients with chronic diseases andundertake clinical medication reviews to proactively manage people with complexpolypharmacy.
Key Areas ofResponsibility
To proactivelymanage people with complex polypharmacy, especially the elderly, people in carehomes, those with co-morbidities (in particular frailty, COPD, and asthma) andpeople with learning disabilities or autism (through STOMP Stop OverMedication Programme).
Deliver patientfacing/remote clinical medical reviews, including long-term conditions clinicsand ensure medicines optimisation. Make appropriate recommendations to GPs, nursesand paramedics for medicine improvement and monitoring.
Work with boththe care home staff and PCN staff to improve the safety of medicines orderingand administration.
To offer support to the Single Point of Access team that supports all the Care Homes in the network.
Attend and/orrefer patients to multidisciplinary care conferences.
Providespecialist expertise in the use of medicines whilst helping to address both thepublic and social care needs of patients at our PCNs Practice.
Providetraining/development to primary healthcare teams on therapeutics and medicinesoptimisation.
Work with the practiceteam to ensure compliance with CQC standards.
