Job summary

Job description

The clinical pharmacist will, as part of a multi-disciplinary team, be ina patient facing role to clinically assess and treat patients usingtheir expert knowledge of medicines for specific disease areas. Within thisrole they would be prescribers,or will be completing training to become prescribers, and will work alongsidethe general practice team.

In additionthey would be expected to take responsibility for the care management of patients with chronic diseases and undertakeclinical medication reviews to proactively manage people with complex polypharmacy, especially theelderly, people in care & nursing homes, those with multiple co-morbiditiesand people with other disabilities.

Main duties of the job

The Clinical pharmacist employed will either be enrolled in or have qualified from an accredited training pathway that equips them to practice and prescribe safely and effectively in a primary care setting.

Fareham & Portchester PCN (FPPCN) Comprises 4 practices. This is Fareham Centre Practice, Gudgeheath Lane Surgery, Westlands Medical Centre, and Portchester Practice. We have a patient population of approximately 47,500 with a diverse workforce of Paramedics, Nurses, ANP, General Practice Assistants, Care Co-ordinators, FCP Physio's, Social Prescribers and a Team of 3 Clinical Pharmacists and 3 Pharmacy Technicians. These are supported by the management and administration teams. We also pride ourselves on the excellent in-house supervision program we have been developing over the past 5 years to ensure maximum support and guidance from the GPs working alongside you. Job responsibilities

37.5 hours per week to include Saturdays 9-2pm (part time hours may be considered)

Within this role they would be prescribers, or will be completing training to become prescribers, and will work alongside the general practice team.

In addition they would be expected to take responsibility for the care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage people with complex polypharmacy, especially the elderly, people in care & nursing homes, those with multiple co-morbidities and people with other disabilities.

To be part of a multi-skilled primary care team providing patient centred medical services based on patient needs with a focus on continuity of care. Including;-

working as part of a multi-disciplinary team in a patient facing role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas.

Being a prescriber, or will be completing training to become prescribers, and will work with and alongside the general practice teams.

taking responsibility for the care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage people with complex polypharmacy, especially the elderly, people in care homes, those with multiple co-morbidities (in particular frailty, COPD and asthma) and people with a learning disability or autism (through STOMP Stopping Over Medication Programme).

providing specialist expertise in the use of medicines whilst helping to address both the public health and social care needs of patients at the PCNs practice(s) and help in tackling inequalities.

providing leadership on person centred medicines optimisation (including ensuring prescribers in the practice conserve antibiotics in line with local antimicrobial stewardship guidance) and quality improvement, whilst contributing to the quality and outcomes framework (QoF) and enhanced services.

through structured medication reviews, supporting patients to take their medications to get the best from them, reduce waste and promote self-care.

having a leadership role in supporting further integration of general practice with the wider healthcare teams (including community and hospital pharmacy) to help improve patient outcomes, ensure better access to healthcare and help manage general practice workload.

developing relationships and work closely with other pharmacy professionals within and across PCNs and the wider health and social care system.

taking a central role in the clinical aspects of shared care protocols, clinical research with medicines, liaison with specialist pharmacists (including mental health and reduction of inappropriate antipsychotic use in people with a learning disability), liaison with community pharmacists and anticoagulation.

Qualifications
  • Pharmacy Degree.
  • Ability to prescribe autonomously, or working towards appropriate qualifications.
  • Experience in undertaking medication reviews including optimal use
  • Primary Care experience.
  • Ability to work as an integrated team member.
  • Participation in CPD, appraisal and clinical governance.
  • Knowledge of current and forthcoming national policy and its effects on clinical pharmacy practice.
  • Self-motivated.
  • Registration with Professional body.
  • Experience in triage.
