An exciting opportunity has arisen for a Clinical Pharmacistto undertake a pioneering role within primary care.
Working as part of a team which currently comprises of threeClinical Pharmacists and a Pharmacy Technician. The PCN is open to applicationsfrom newly qualified Pharmacists.
You will work closely with all PCN and practice staff.
To work as part of a multi-disciplinary team in apatient-facing role.
Provide clinical leadership on medicines management andquality improvement as well as managing some aspects of the quality andoutcomes framework and enhanced services.
Primary Duties and Areas of Responsibility includes but notlimited to: Long-term condition Clinics, Structured Medication Review,medicines support, medicine information to practice staff and patients,unplanned hospital admissions reviews, management of medicines at dischargefrom hospital, repeat prescribing, risk stratification, service development,information management, medicines quality improvement, medicines safety,implementation of local and national guidelines and formulary recommendations.

We are interested to hear from candidates looking for fullor part-time roles to fill our vacancies. All applicants will be required tohold current GPhC registration as Pharmacist. Previous experience of working ina GP practice is desirable but not essential. Applicants will need to becomputer literate, confident in working independently, able to prioritiseworkload and want to work in a multi-disciplinary environment.
