PCN Clinical Pharmacist

An exciting opportunity has arisen for a Clinical Pharmacistto undertake a pioneering role within primary care.

Working as part of a team which currently comprises of threeClinical Pharmacists and a Pharmacy Technician. The PCN is open to applicationsfrom newly qualified Pharmacists.

You will work closely with all PCN and practice staff.

Main duties of the job

To work as part of a multi-disciplinary team in apatient-facing role.

Provide clinical leadership on medicines management andquality improvement as well as managing some aspects of the quality andoutcomes framework and enhanced services.

Primary Duties and Areas of Responsibility includes but notlimited to: Long-term condition Clinics, Structured Medication Review,medicines support, medicine information to practice staff and patients,unplanned hospital admissions reviews, management of medicines at dischargefrom hospital, repeat prescribing, risk stratification, service development,information management, medicines quality improvement, medicines safety,implementation of local and national guidelines and formulary recommendations.

Previous experience of working ina GP practice is desirable but not essential. Applicants will need to becomputer literate, confident in working independently, able to prioritiseworkload and want to work in a multi-disciplinary environment.

Experience
  • -Mandatory registration with General Pharmaceutical Council
  • -Membership of the Royal Pharmaceutical Society
  • -A member of or working towards Faculty membership of the Royal Pharmaceutical Society
  • -Professional Indemnity Insurance is required and will be reviewed on an annual basis.
  • -Independent prescriber or
  • -working towards/intent of
  • -Minimum of 2 years post-qualification experience
  • -In depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare.
  • -An appreciation of the nature of GPs and general practices.
  • -An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing.
  • -Excellent interpersonal, influencing and negotiating skills.
  • -Excellent written and verbal communication skills.
  • -Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences (eg, patients).
  • -Recognises priorities when problem solving and identifies deviations from the normal pattern and is able to refer to seniors or GPs when appropriate.
