PCN Care Coordinator - Modality Walsall West One

Your job is to work directly with key stakeholders to help us to harmonise ways of working and improve working practices to enhance patient and staff satisfaction.

You will be the key link to the people whose care you are supporting, operating as a go-to person to ensure seamless care, active decision-making by patients, and effective collaboration among all involved.

Care coordinators provide extra time, capacity, and expertise to support patients in preparing for or following up on clinical conversations with primary care professionals.
