A healthcare organization in Birkenhead is seeking a part-time PCN Care Coordinator. You will work 20 hours a week and coordinate patient care within a supportive clinical team. Ideal candidates have experience in primary care and the ability to effectively manage patient care plans. We are seeking a care coordinator to join our PCN team, working 20 hours a week across five days (Monday-Friday). The successful applicant will work closely with the Clinical Pharmacist team to coordinate timely patient care.
The care coordinators role requires them to beable to work with, and understand the roles of, a variety of different peopleworking in a practice and across the PCN including doctors, nurses, healthcareassistants, social prescribing link workers, physiotherapists, pharmacists,health and wellbeing coaches and pharmacy technicians.
Work closely with practice and PCN healthcareroles, the carecoordinator is towork within the GPCP Team acting as first point of contact
Collate all of a patients identifiedcare and support needs and review the options to meet these needs and bringthem into a single personalised care and support plan (PCSP) in line with bestpractice
Supportpeople to manage their needs by answering their queries and supporting them inmaking appointments or accessing appropriate services
Supportthe clinical team to engage with wider health professionals to coordinate care
Managingreferrals and ensuring the patient understands and inputs into the care plan
Assistpatients to be better prepared to have conversations on shared decision makingand to improve awareness of shared decision making and related support tools
Providepatients with high quality, easy to understand information to assist them inmaking choices about their care
Liaisewith other care coordinators in other practices within the PCN and share bestpractice
Providecoordination and navigation of patients, and where appropriate their carers,across health and social care services
Supportin the delivery of enhanced services and other service requirements on behalfof the PCN
BrighterBirkenhead PCN is a group of 6 GP practices in and around the Birkenhead areaof Wirral, working together to help patients live healthier, happierlives. We have an exciting opportunityfor Care Coordinator to join our Medicines Management Team, working alongside our Clinical Pharmacists.
Job Summary
A care coordinator is required to deal withpatients and, if appropriate, their carer, before or after the patientsconsultation with a clinician or other healthcare professional.
Working closely with the patient and theirclinician and/or other healthcare professionals, the care coordinatorco-ordinates patients healthcare and supports them to access the appropriateservice to ensure that they get the most suitable care from whatever health orsocial care provider is appropriate.
The care coordinators role requires them to beable to work with, and understand the roles of, a variety of different peopleworking in a practice and across the PCN including doctors, nurses, healthcareassistants, social prescribing link workers, physiotherapists, pharmacists,health and wellbeing coaches and pharmacy technicians.
They will be given a caseload of identifiedpatients and be required to ensure that their changing needs are addressed bytaking into account local priorities, health inequalities and/or populationhealth management risk stratification. This role will specifically work withinthe GP Clinical Pharmacist (GPCP) Team and with patients on significant orcomplex medication regimes on behalf of all 7 member practices but may shift onto other projects as services develop.

Providecoordination and navigation of patients, and where appropriate their carers,across health and social care services
Supportin the delivery of enhanced services and other service requirements on behalfof the PCN
Leadin the management of patient complaints and participate in the identificationof any necessary learning brought about through clinical incidents andnear-miss events
Organise,attend and participate in the delivery of multi-disciplinary teams (MDT) withinPCNs.
Undertakeall mandatory training and induction programmes
Contributeto and embrace the spectrum of clinical governance
Developyourself and the role through participation in training and service redesignactivities
Attenda formal appraisal with their manager at least every 12 months. Once a performance/trainingobjective has been set, progress will be reviewed on a regular basis so thatnew objectives can be agreed.
Contributeto public health campaigns (e.g. flu clinics) through advice or direct care
Maintaina clean, tidy, effective working area at all times
Coordinatecare and support for patients on significant or complex medications
Actas first point of contact for queries as they come, looking out for trendsacross the practices
Managethe team rota to ensuring each practice gets its fair share of allocated time
Measureoutcomes and monitor reports
Managetimely comms between the GPCP team and the 7 PCN Practices
Developpatient communications
Keepabreast of what is going on locally/nationally (i.e. PQRS, DES & QOF)
Workingwith the practices to understand the individual need
Facilitatea Brighter Birkenhead network for medicines admin colleagues, developinglearning opportunities and sharing best practice
Communicatewith local pharmacies.
Workwith the GPCP to create processes and procedures that streamline patientpathways
Workwith the GPCP and other services to create links and pathways for examplewellbeing practitioners, health coaches
Tosupport patient education around medicine compliance
