A healthcare provider in Liverpool is seeking a Care Coordinator to support patients' personalized care needs. Responsibilities include coordinating care with multi-agency organizations and supporting national screening initiatives. North Liverpool Primary Care Network is seeking to recruit a Practice based Care Coordinator, to undertake work in line with the Primary Care Network directed priorities. The role will be based within Aintree Park Group Practice, which has 2 sites, Moss Lane surgery in Orrell Park, Liverpool and the Oriel Drive surgery, Aintree.
The ideal candidate will have previous experience of ability to work under pressure and own initiative. The ability to work as part of a team is essential.
Excellent communication skills and the ability to establish and maintain effective working relationships and promote collaborative practice with all colleagues from all dimensions within and the wider range of the PCN.
Previous experience of working within a healthcare field (NHS) or with other healthcare professionals, and general practice is essential
Aintree Park Group Practice is a well established GP Practice based within the north of the city. The Practice is made up from the Partners, Operations Manager, Clinical Teams supported by administration teams.
Hours of work will be 37.5 hours per week, Monday - Friday between the operating hours of 7.30am - 8pm with a daily unpaid break of 30 mins.
25 days Annual leave per year.
Key responsibilities:
Undertake practice work in line with PCN directed priorities
Proactively identify and work with a cohort of practice patients to support their personalised care requirements
Support uptake of national screening programmes and immunisation programmes
Direct liaison with multi agencies to coordinate care for patients
Support quality and outcome frameworks and other DES/LES specifications
Support the Practice with Learning Disability and Cancer patient targets
Provide coordination and navigation for people and their carers across health care services alongside working closely with the social prescribing link workers and primary care roles
