Job description

Chorley Central Primary Care Network are looking for an experienced Care Co-ordinator to join the PCN Care Home team.

You will be part of an established team delivering the Enhanced Care in Care Home (EHCH) service for the 16 Care Homes in the PCN.

We are looking for someone who is passionate about delivering person-centred care, has a professional approach to their work, and is caring, dedicated, and compassionate to the needs of the patient.

Main duties of the job

This role will be based at Regent House Surgery and will involve traveling between care homes.

Working as part of a well-established team and strictly in accordance with PCN EHCH Guidelines and protocols.

Conducting 6-monthly health and care plan reviews.

Ensuring all residents have a Personalised Care and Support Plan (PCSP).

Supporting the team in administering Influenza and COVID vaccinations.

Providing administrative support for the weekly MDT meetings.

Hours are 9am - 2pm, Monday to Thursday.

Job responsibilities

Your duties will include:

  • Conducting 6-monthly health and care plan reviews.
  • Ensuring all residents have a Personalised Care and Support Plan (PCSP).
  • Supporting nurses in administering Influenza and COVID vaccinations.
  • Providing administrative support for the EHCH Team.
  • Maintaining accurate clinical records in accordance with current legislation.
Person Specification
Knowledge
  • Understanding of systems to assess the health needs of the population related to primary care.
  • Understanding of evidence-based practice.
  • Knowledge of national standards that inform practice (e.g., National Service Frameworks, NICE guidelines).
  • Understanding policy developments related to primary care services, including General Practice, GMS/PMS contracts, Clinical Governance, and the QOF.
Qualifications
  • Experience working in health, social care, or related roles involving direct contact with people, families, and carers.
  • Experience in performing and recording basic health measures (e.g., blood pressure, oxygen saturation, respiratory rate, weight).
  • Experience handling confidential and sensitive information.
  • Ability to work independently, organize, and prioritize workload.
  • Excellent communication skills, both written and verbal, with strong time and case management abilities.
  • Car driver or access to a vehicle.
  • NVQ Level 2 in Health and Social Care or equivalent.
  • Experience working with older, vulnerable patients or adults with complex needs.
  • Experience with EMIS or other medical software.
