As a Care Coordinator, you'll play a vital role in improving patient care by supporting people, especially the elderly and those with long-term conditions, to navigate the health and care system with confidence.

Main duties of the job
  • Work closely with GPs, practice staff, and wider PCN teams
  • Act as the main point of contact for patients and carers
  • Help develop and coordinate personalised care plans
  • Refer to services like social prescribing and wellbeing coaching
  • Support patients before and after clinical appointments
  • Empower individuals to understand and manage their health

This is not a clinical role, but it's central to delivering high-quality, joined-up care that puts people first.
