Birmingham Community Healthcare NHS Foundation Trust is seeking clinicians to undertake same-day and urgent care appointments, assess and manage patients, perform home visits when indicated, and order investigations within their scope.

You will provide health promotion, maintain records in EMIS Web, work with a multidisciplinary team, and collaborate with GPs, nurses, pharmacists and care teams to deliver high-quality care.

Responsabilités

• Undertake same-day and urgent care appointments.
• Assess, diagnose, treat and refer patients appropriately.
• Conduct home visits and care home visits where clinically indicated.
• Request and interpret appropriate investigations within scope of practice.
• Provide health promotion and self-management advice.
• Maintain accurate records using EMIS Web and other approved systems.
• Work collaboratively within the multidisciplinary team.
• Manage acute presentations.
• Undertake medication reviews within competency.
• Recognise deterioration and elevate appropriately.
• Participate in safeguarding processes.
• Work closely with GPs, nurses, pharmacists, social prescribers, care coordinators, mental health practitioners and community teams.
