PCN Advanced Nurse Practitioner / Advanced Paramedic Practitioner
Overall, the post holder will work collaboratively with the PCN team to meet patient needs, implement policies and procedures, and provide nurse leadership as required.
Key Responsibilities
- Work in accordance with the relevant Code of Conduct and act as a positive role model to staff and patients.
- Work as an autonomous practitioner, attending to patients, diagnosing, treating, and prescribing for undifferentiated and undiagnosed illnesses or injuries.
- Assess patient needs based on highly developed clinical skills, including physical assessment.
- Provide advice on ongoing medical and nursing needs.
- Administer treatments within protocols, policies, and clinical guidelines.
- Review medication for therapeutic effectiveness, ensuring practices are evidence-based and within clinical guidelines.
- Maintain competencies and work within your scope of practice, adhering to clinical protocols.
- Participate in setting standards of patient care based on research findings.
- Complete required online and face-to-face training to maintain competence and professional development.
- Demonstrate flexibility and innovation in working methods to improve access for patients.
- Maintain confidentiality and trust, especially when working with vulnerable groups.
- Ensure clear, contemporaneous documentation of patient care records.
- Maintain a portfolio for NMC revalidation to uphold professional registration.
- Provide high-quality advanced clinical care to a defined practice population.
Clinical Role
- Identify, assess, and diagnose patients with complex health needs across the community, including in residential and nursing homes.
- Lead the introduction and use of technical and specialist nursing treatments.
- Prescribe products to meet patient needs within the PCN.
- Develop programs to improve health and well-being, addressing community, group, and individual needs.
- Work with others to influence policies that improve health and reduce inequalities.
- Mentor clinical staff where competencies allow.
- Implement and evaluate evidence-based practices to enhance services.
- Participate in quality assurance, development, and clinical governance.
- Collaborate with team members on national guideline implementation.
- Adopt a holistic approach to promote health through preventative measures.
Administrative and Professional Responsibilities
- Ensure accurate and legible documentation of consultations and treatments.
- Keep the clinical computer system updated with accurate patient details.
- Complete all necessary documentation for patient care and PCN registration.
- Maintain statistical data for reports and audits.
- Participate in PCN meetings as required.
Personal and Professional Development
- Engage in ongoing training, monitored through yearly appraisals, in line with PCN requirements.
- Stay up-to-date with recent developments in practice.
- Participate in training and education of students and new staff members.
- Develop and maintain an in-house nurse-training program.
- Attend courses and study days to meet PREP requirements.
- If role expansion occurs, receive full training for additional responsibilities.
- Develop and maintain a Personal Learning Plan.
Other duties may be assigned as needed to meet patient and PCN needs.
