We are looking for an Advanced Nurse Practitioner or an Advanced Paramedic Practitioner to join our growing PCN team, to help support diagnosing and treating patients presenting with minor illness / acute conditions, seeing and advising people in respect of their continuing medical and nursing needs including liaison with internal and external professionals and other agencies to manage the patient effectively and efficiently.

Overall the post holder will work collaboratively with the PCN team to meet the needs of the patients, delivery of policies and procedures and providing nurse leadership as required.

Responsible for attending to patients who wish to be seen by a health care professional, diagnosing, treating and prescribing for patients presenting with undifferentiated and undiagnosed illness or injury making an assessment of their needs, based on highly developed clinical skills including physical assessment.
Seeing and advising people in respect of their continuing medical and nursing needs.
Administer treatment within protocols, policies and clinical guidelines.
Review medication for therapeutic effectiveness appropriate to the patients needs in accordance with evidence-based practice and clinical guidelines.
To maintain competencies associated with the role ensuring you are working within your own limitations / scope of practice.
Ensure clinical protocols are always adhered to.
To participate in the setting of standards of patient care based on sound research findings.
To complete on-line and face to face training as required, maintaining competence and professional development.
Demonstrate flexible and innovative ways of working to provide access to the patient population.
Always maintain confidentiality and trust.
Adhere to consent and understand the working principles involved, especially in relation to vulnerable groups.
Clear contemporaneous recording and updating patient care records.

To provide high quality advanced clinical care to a defined practice population. The role will include responsibility within the PCN for the development and direction of the clinical and managerial aspects of nursing care / paramedic care, as well as an advanced clinical role. The role is flexible and demanding and may be expanded in accordance with PCN requirements and the nurse practitioners own scope of practice.
Clinical Role
Identify, assess and diagnose patients (adults and children) with complex health needs across the PCN population including in the community, Residential and Nursing Homes
Lead the introduction and use of technical and specialist nursing treatments for patients with complex healthcare needs.
Prescribe products to meet the needs of patients within the PCN population.
Lead and develop programmes to improve health and well-being and meet the specific needs of communities, groups and individuals.
Work with others to change policies to improve health and well-being and reduce inequalities.
Mentor clinical staff where competencies allow.
The implementation and evaluation of evidence-based practice to improve the services offered by the PCN nursing team.
Participate in quality assurance, development and clinical governance.
Work with other members of the PCN team in the implementation of specific aspects of national guidelines.
To adopt a holistic approach working with individuals and wider populations to promote health through preventative measures.
Administrative and professional responsibilities
Ensure accurate and legible notes of all consultations and treatments are recorded in the patients notes.
Ensure the clinical computer system is kept up to date, with accurate details recorded and amended.
Ensure accurate completion of all necessary documentation associated with patient health care and registration with the PCN.
Ensure collection and maintenance of statistical information required for regular and ad hoc reports and audit.
Attend and participate in PCN meetings as required.
Personal and Professional Development
Training requirements will be monitored by yearly appraisal and will be in accordance with PCN requirements. Personal development will be encouraged and supported by the PCN. It is the individuals responsibility to remain up to date with recent developments.
Participate in the education and training of students of all disciplines and the introduction of all members of the PCN staff where appropriate.
Develop and maintain an in-house nurse-training programme reporting to the operations manager.
Maintain continued education by attendance at courses and study days as deemed useful or necessary for professional development, ensuring PREP requirements are met.
If it is necessary to expand the role to include additional responsibilities, full training will be given.
Develop and maintain a Personal Learning Plan
