The GP Assistant supports effective servicedelivery within the Primary Care Network by organising and streamlining carecoordination processes, managing administrative systems, and ensuring smoothcommunication between teams. This role involves proactively supporting patientsand staff with complex needs through logistical planning, working closely withpractice teams, external providers, and the wider PCN to ensure services aredelivered efficiently and experiences are improved. The GP Assistant is responsible for managing administrative and coordination tasks across the PCN, including rota management, meeting organisation, data handling, and inter-agency communication. The role supports smooth information flow between services, maintains accurate records using systems like EMIS, assists with audits and reporting, organises events and training logistics, and ensures compliance with data protection standards. The postholder also plays a key role in improving internal workflows, preparing documentation, and supporting the wider PCN team with service development and operational delivery.
We are a Primary Care Network (PCN) of five practices working togetherwith a range of local providers, including primary care, community services,social care, and the voluntary sector, to offer more personalized, coordinatedhealth and social care to their local populations.
Our PCN comprises of 35k+ patients over the five practices, making uslarge enough to provide resilience and support the development of integratedteams.
