Job overview

This role involves working closely with the Senior Patient Safety Advisor, Patient Safety Manager, Head of Clinical Governance, and Deputy Chief Medical Officers to identify and manage incidents, participate in weekly meetings, and consider learning response options under the Patient Safety Incident Response Framework (PSIRF). The appointee will collaborate with colleagues across the Trust to ensure systems support a culture of open reporting and investigation. Responsibilities include supporting divisional governance teams, developing and delivering training, and reviewing learning outcomes to identify systemic issues and prevent reoccurrences. The post-holder will work under the guidance of the Senior Patient Safety Advisor.

Main duties of the job

As previously described, including incident evaluation, managing safety meetings, maintaining logs, liaising with various departments, and ensuring effective communication and documentation related to patient safety.

Specific Responsibilities

  • Evaluate the impact of incidents.
  • Coordinate weekly Safety, Learning & Improvement Conversation (SLIC), including agenda setting, administration, and documentation.
  • Attend patient safety response meetings twice weekly and document outcomes.
  • Maintain investigation logs.
  • Collaborate with Legal Services, Complaints, Tissue Viability, and others for effective communication about incidents.
  • Participate in weekly triangulation meetings involving legal cases, safeguarding, and feedback.
  • Ensure communication with local Health & Safety regarding incident reporting.
  • Coordinate with NHS England and other bodies to share documents and respond to FOI requests.
  • Provide evidence for Duty of Candour compliance.
