Job summary

The Royal Surrey NHS Foundation Trust is seeking a Patient Safety Midwife to join the Women and Children’s Clinical Governance team in Guildford. You will support the governance agenda, attend key meetings, and ensure accurate minutes while disseminating learning across maternity services.

You will provide expert advice on patient safety, coordinate incident investigations across Maternity, Gynaecology and Paediatrics, and support PSIRF and reporting requirements.

Responsibilities

• Attend and contribute to meetings (Maternity Quality and Safety, Perinatal Morbidity and Mortality); ensure minutes are completed and learning disseminated.
• Provide expert advice and facilitation for patient safety governance activities across the Women and Children's Division.
• Coordinate incident reviews across Maternity, Gynaecology and Paediatrics; liaise with PSIRF and other reporting requirements (MBRRACE, NHS Resolution, Early Notification).
• Deliver specialist training in clinical governance and incident reporting/investigation to staff across the division.
• Contribute to audits, guideline management, document review and respond to complaints and patient feedback.

Job description

The post holder will be part of the Women and Children\'s Clinical Governance team, which specifically supports the Trust\'s governance agenda. They will be responsible for attending and contributing to a range of meetings, including Maternity Quality and Safety, Perinatal Morbidity and Mortality with the purpose of ensuring completion of the minutes and disseminating learning throughout the maternity department.
The post holder will provide expert advice, practical support and facilitation of a wide range of patient safety, governance and improvement activities. The post holder will develop and maintain effective working relationships with members of the Women and Children\'s Division to ensure incidents are identified, reported and investigated, and that shared learning and improvements are implemented using PSIRF., Working as part of the Women and Children\'s Clinical Governance team, the Patient Safety Midwife will be responsible for providing clinical governance expertise to support the Patient Safety Incident Response Framework Policy (PSIRF) and Trust\'s Plan. The Patient Safety Midwife will primarily be responsible for, working collaboratively with a wide range of staff, reviewing all incidents within the division (across Maternity, Gynaecology and Paediatrics) coordinating patient safety event investigations, creating actions and providing assurance regarding their completion.
The Patient Safety Midwife will include ensuring the compassionate engagement and effective communication with all members of the clinical and non-clinical team, patients, carers and relatives. They will support the weekly Divisional Patient Safety Incident Response Group and work with the Women and Children\'s Clinical Governance team to coordinate the effective review and proportionate response to incidents and safety issues. They will use their expertise and skills in investigating and analyzing incidents to write incident reports and provide reports to the Division and the Division\'s SBUs. They will support the completion of the Perinatal Mortality Review Tool within the Division, and will be required to work with the Maternity and Neonatal Safety Investigation Team and ensure reporting requirements to MBRRACE, NHS Resolution and the Early Notification Scheme are met., The post holder will provide specialist training in clinical governance across the division for all levels of staff. This will include training in relation to incident reporting, incident investigation and meet with staff to share learning from incidents.
The post holder will also be responsible for undertaking audit, managing guidelines and the document review process and other clinical governance activities, such as responding to complaints and patient feedback and supporting the running of meetings. They will be responsible for deputizing for the Head of Clinical Governance (Women and Children\'s division) in their absence. The post holder will contribute to the wider management of the service, such as supporting staff to undertake quality improvement projects. They will also be required to work clinically at times of high acuity and be a part of the midwifery on-call rota.
