South London and Maudsley NHS Foundation Trust is seeking a patient safety governance professional to support clinical teams in identifying factors contributing to incidents and system improvements using quality improvement methods and human factors.
You will oversee maternity incident reporting, ensure data quality in electronic risk management systems, and prepare regular safety reports for governance meetings and the Trust Board.
Support clinical teams to identify contributory factors and system improvements arising from patient safety incidents, using quality improvement methodologies and human factors principles to strengthen patient safety and reduce recurrence. Maintain robust oversight of maternity incident reporting, ensuring the quality, completeness and accuracy of incident data and supporting the effective use of electronic risk management systems. This is for the purpose of monthly maternity governance meeting and relevant divisional meetings, and this includes gynaecology as directed by patient safety manager in her absence. Analyse incident data, identify emerging trends, themes and areas of concern, and prepare regular patient safety reports and dashboards for the Maternity Governance Committee, Divisional Governance structures and Trust Board, working collaboratively with Business Intelligence and data teams as required. Monitor the implementation and effectiveness of recommendations arising from patient safety investigations, MNSI reports, complaints, inquests and national safety alerts, ensuring learning is embedded into clinical practice and improvements are sustained. Ensure learning and outcomes are disseminated across all areas.
