Job description

The specialist division comprises 13 specialties that operate trust-wide, offering a range of services. This role provides an opportunity to collaborate across the entire multidisciplinary team (MDT) within the division to develop our patient safety portfolio. It is a new position created to support the development of a quality and safety team within the division.

The Patient Safety Investigator will investigate patient safety incidents in accordance with the Patient Safety Incident Response Framework (PSIRF) and in line with Trust policies and procedures. The postholder will identify areas for improvement, recommend actions to enhance safety, and implement safety protocols.

Patient safety investigators will promote a culture of safety by focusing on continuous improvement. They will investigate factors influencing patient safety incidents, including human factors, teamwork, equipment, workspace, culture, and organizational drivers, and will recommend improvements.

The role requires a combination of clinical knowledge, analytical skills, and understanding of patient safety standards to effectively investigate incidents, draw conclusions, and facilitate improvements and action plans. Engagement with patients, families, divisional leadership, and clinical teams is essential.
