Job description

An exciting opportunity has arisen for an enthusiastic, quality-driven Patient Pathway Coordinator to join our busy and dynamic Elective Pathways team.

We are seeking individuals with excellent attention to detail and the confidence to communicate effectively at all levels. The ideal candidate will maintain high standards, manage and prioritize their workload, remain calm under pressure, and enjoy working as part of a team.

It is desirable that candidates are familiar with NHS patient administration systems and have a working knowledge of local and national access targets.

Our vision is to develop and deliver a high-quality Elective Pathways Service across the Northern Care Alliance, aiming to recruit and retain motivated, high-quality staff in a modern, fit-for-purpose structure that provides job satisfaction and work-life balance.

Please note that while this post is based at Rochdale Infirmary, there is an opportunity to work from home. However, occasional travel across NCA sites may be required.

Key Responsibilities:
  1. Facilitate a timely patient journey by ensuring coordinated processes centered around patient needs.
  2. Ensure potential breaches of RTT (referral to treatment) and cancer waiting times are avoided or escalated appropriately.
  3. Assist in developing administrative systems to streamline patient pathways and improve care quality.
  4. Provide cross-cover for other Patient Pathway Coordinators to maintain effective administration services.
  5. Provide Directorate Managers with performance information related to Access Targets after exploring all solutions.
  6. Identify capacity shortfalls and escalate resource requirements.
  7. Maintain professionalism and high customer care standards.
  8. Participate in continuous learning and departmental meetings.

The Northern Care Alliance NHS Foundation Trust provides hospital and social care services to over one million people across Greater Manchester. Our 20,000 colleagues work in hospitals and communities across Bury, Rochdale, Oldham, and Salford to save and improve lives.

We are committed to high standards of care, working closely with local authorities and partners. We value our core principles: care, appreciation, and inspiration. Join us to make a difference.

We offer roles with purpose and flexibility, with opportunities for career development. Benefits include a competitive leave allowance, flexible working, and protected time for health and wellbeing activities.
